dc.description.abstractDas Kolorektale Karzinom (KRK) gehört zu den häufigsten Krebserkrankungen des Menschen. Die Mortalität in Deutschland liegt bei Männern und Frauen bei 26 000 und die 5-Jahres-Überlebensrate beträgt 60% und ist damit nach wie vor inakzeptabel hoch. Daher sind weitere Verbesserungen in der Diagnostik und der Therapie dieser Erkrankung dringend notwendig. Es existieren zwar bereits einige Tiermodelle zum KRK, jedoch basieren diese meist auf der Injektion von Zelllinien und die Ergebnisse dieser Modelle lassen sich nur bedingt auf den Menschen übertragen. Das hier etablierte genetisch induzierte Mausmodell (GEMM) zeichnet sich dadurch aus, dass es durch gewebsspezifische Genveränderungen die Tumoren direkt in der Maus entstehen lässt. Im hier beschriebenen Modell wurden Mutationen nicht nur im Apc-Gen und im Kras-Gen sondern erstmals auch im Tp53- Gen im Kolon der Maus induziert. Die Kombination dieser drei Mutationen kommt im humanen KRK häufig vor und ist maßgeblich an der Adenom-Karzinom-Sequenz beteiligt. Zur gewebsspezifischen und zeitlich gesteuerten Aktivierung der Mutationen wurde das Cre/loxP-System verwendet; die konditionellen Mutationen in der Keimbahn wurden durch lokale Infektion des Kolons mit einem Cre-Adenovirus aktiviert. Verschiedene Mauslinien wurden gezielt verpaart, um die folgenden Genotypen zu erhalten: Apcfl/fl ( Apc ), Apcfl/fl / KrasLSL-G12D/+ ( AK ), Apcfl/fl / KrasLSL-G12D/+ / Tp53fl/fl ( AKPfl ) und Apcfl/fl / KrasLSL-G12D/+ / Tp53LSL-R172H/+ ( AKPr ). Zur zusätzlichen Untersuchung der onkogenen Funktion von Punktmutationen im Tp53-Gen wurde nicht nur ein konditionelles Knockout-Allel (Tp53 fl/fl), sondern auch eine konditionelle Hotspot-Mutante von Tp53 (LSL-Tp53 R172H) untersucht. Nach der Infektion mit dem Adenovirus entwickelten 83% (n=207) der Tiere innerhalb weniger Wochen Tumoren im infizierten Darmsegment. Das Tumorwachstum wurde durch repetitive Koloskopien überwacht und die Tumoren wurden zu drei verschiedenden Zeitpunkten (56d,112d und 168d) bei jeweils acht Tieren von jedem der vier Genotypen (n=74) entnommen und histologisch untersucht. In einer weiteren Kohorte wurde das Überleben abhängig vom Genotyp der Tumoren bei 60 Tieren gemessen.Wie erwartet entwickelten und entdifferenzierten sich die Tumoren der AK-Mäuse schneller als jene der Apc-Mäuse. Nach 8 Wochen zeigte ein Tier (n=1; 14%) der Apc-Mäuse das Wachstum eines mikroinvasiven Karzinoms, wobei bei den AK-Mäusen nach dieser Zeit bereits die doppelte Anzahl (n=3; 38%) an mikroinvasiven Karzinomen beobachtet wurde. Bei den AKPfl- und AKPr-Mäusen resultierte sowohl die Punktmutation in Tp53 als auch der vollständige Verlust von Tp53 in rasch wachsenden entdifferenzierten Tumoren. Nach 24 Wochen wurde bei fast allen (n=4; 80%) AKPf-Mäusen und bei dem Großteil (n=6; 86%) der AKPr-Mäuse die Entwicklung zu invasiven Karzinomen analysiert. Tiere mit einem vollständigen Tp53-Verlust zeigten insgesamt ein aggressiveres Tumorwachstum verglichen mit Mäusen mit einer Punktmutation in Tp53. Diese Beobachtungen ließen sich auch durch das histologische Grading bestätigen. Während bei den weniger komplexen Genotypen (Apc, AK) nicht-invasive Adenome und gut differenzierte Adenokarzinome auftraten (n=33; 87%), zeigten sich in den Tieren mit Tp53-Veränderungen mehrheitlich (n=26; 72%) schlecht bis entdifferenzierte Karzinome.Fernmetastasen (n=5; 4%) traten nahezu ausschließlich bei den AKPr-Mäusen auf, was vermuten lässt, dass das verglichen mit den AKPfl-Mäusen langsamere Wachstum zusammen mit der gewonnenen onkogenen Eigenschaft die Metastasenbildung begünstigt. In der Überlebensanalyse zeigte sich ein deutlicher Zusammenhang zwischen höhergradig komplexen Genotypen und dem Überleben der Tiere. Die kürzeste Überlebenszeit zeigten AKPfl-Mäuse, bei denen mehr als die Hälfte (n=8; 53%) an Lokalkomplikationen des schnellen Tumorwachstums verstarben.Insgesamt konnte gezeigt werden, dass im hier vorgestellte Mausmodell murine kolorektale Karzinome entstehen, die sowohl genetisch als auch klinisch dem humanen Tumorwachstum nahezu identisch sind. Damit sind weitere Forschungsarbeiten möglich, um noch mehr über diese Tumorerkrankung zu erfahren um neue Therapieansätze zu entwickeln, die langfristig die Mortalität senken könnten.de_DE
dc.description.abstractThe colorectal cancer (CRC) counts among the most frequent cancer diseases in humans. The mortality in Germany is 26,000 for men and women, and the 5-year survival rate is 60%, which is still unacceptably high. Therefore, further improvements in diagnostics and the therapy of this disease are urgently necessary. In fact, there are already existing some animal models concerning the colorectal cancer, but most of them are based on the injection of cell lines and the results of these models can only conditionally transferred to humans. One of the characteristics of the established genetically engineered mouse model (GEMM) is that the tumours are grown directly in the mouse by tissue-specific gene mutation. In the described model the modifications have not only been induced in the Apc gene and in the Kras wild type gene, but for the first time they have also been induced in the Tp53- gene in the colon of the mouse. The combination of these three mutation types is a frequent case in human CRC and is substantially involved in the adenoma-carcinoma sequence. For the tissue-specific and time-managed activation of the mutations the Cre/IoxP-system has been used; the conditional mutations in the germ line have been activated by local infection of the colon with a Cre-adenovirus. In order to get the following gene types, different mouse lines have been aimingly paired: Apcfl/fl ( Apc ), Apcfl/fl / KrasLSL-G12D/+ ( AK ), Apcfl/fl / KrasLSL-G12D/+ / Tp53fl/fl ( AKPfl ) and Apcfl/fl / KrasLSL-G12D/+ / Tp53LSL-R172H/+ ( AKPr ). For the additional study of the oncogenic function of point mutations in the Tp53 gene there was not only a conditional Knockout-Allel (Tp53 fl/fl) investigated, but also a conditional hotspot mutant of (LSL-Tp53 R172H). After infecting the animals with the adenovirus 83 % (n=207) developed tumours in the infected intestinal segment. The growth of the tumours have been observed by repetitive colonoscopies and the tumours have been extracted from each of the four genotypes (n =74) and histologically examined at three different specific times (56d, 112d and 168d). The survival subject to the tumours genotype has been measured in 60 animals using a further cohort survey. As expected, the tumours of the AK mice developed and dedifferentiated faster than those of the Apc mice. After 8 weeks, an animal (n =1; 14%) of the Apc mice showed the growth of a microinvasive carcinoma, with twice as many (n =3; 38%) microinvasive carcinomas being observed in the AK mice after this time. In case of the AKPf and AKPr mice, the point mutation in Tp53 as well as the total loss of Tp53 resulted in fast growing dedifferentiated tumours. After 24 weeks, the development to invasive carcinomas was analyzed in all (n = 5; 100%) AKPf mice and in the majority (n =5; 71%) of the AKPr mice. Compared to those mice with a point mutation in Tp53 the animals with a total loss of Tp53 showed all in all a more aggressive growth of the tumours. These observations could also be confirmed by the histological grading. While in case of the less complex genotypes (Apc, AK) non-invasive adenomas and well differentiated adenocarcinomas appeared (n=33; 87%), the animals with Tp53 modifications showed majority (n=26; 72%) poorly to dedifferentiated carcinomas. Remote (distant) metastases (n=5; 4%) appeared almost exclusively in AKPr-mice, which leads to the assumption that the - in comparison to the AKPf mice - slower growth in combination with the oncogenic characteristics favours the formation of metastases. The survival analysis showed a clear correlation between higher graded complex genotypes and the survival of the animals. The AKPfl mice showed the shortest survival time, more than the half (n=8; 53%) died due to local complications due to the fast growing tumours. All things considered it could be shown that in the introduced mouse model murine colorectal carcinoma emerge which are genetically as well as clinically almost identical to the human tumour growth. Thus, further research to get to know more about these tumour diseases is possible in order to develop new therapy approaches which could lower the mortality in the long run.en
