dc.description.abstractIn der vorliegenden Arbeit wurde die Prävalenz sowohl gegen endokrines als auch gegen exokrines Pankreasgewebe gerichteter Autoimmunantikörper bei Patienten mit einer chronischen Pankreatitis mit und ohne Manifestation eines Diabetes mellitus, bei Patienten mit verschiedenen Diabetesformen und Kontrollpersonen untersucht. Ziel der Untersuchung war herauszufinden, ob es ein kombiniertes Auftreten pankreatischer endokriner und exokriner Antikörper bei Patienten mit einer chronischen Pankreatitis gibt, welches hinweisend ist für eine immunologische Antwort sowohl gegen exokrines als auch gegen endokrines Pankreasgewebe, hervorgerufen durch eine exokrine Inflammation des Pankreas. Analysiert wurden jeweils Seren von 48 Patienten mit CP, 48 Patienten mit Dm1, 48 Patienten mit Dm2 sowie von 28 Kontrollpersonen. Untersucht wurde auf Inselzell- Antikörper (ICA-Ak) mittels indirektem Immunfluoreszenz-Verfahren, Glutamat Decarboxylase 65 Antikörper (GAD-Ak) und Tyrosin-Phosphatase IA2-Antikörper (IA2-Ak) mittels Radioliganden-Assay, allesamt Autoimmunantikörper, deren korrespondierende Antigene in den Insellzellen des Pankreas zu finden sind und als immunologischer Marker im autoimmunen Diabetes ihre Anwendung finden. Zudem wurde nach karbonische Anhydrase II-Antikörpern (CAII-Ak) und Lactoferrin-Antikörpern (LAC-Ak) mittels ELISA gesucht. Diese Antikörper gelten als immunologische Marker bei exokriner Autoimmunität. Die Ergebnisse der Analysen lassen sich wie folgt zusammenfassen: Ein kombiniertes Auftreten hinsichtlich der untersuchten Antikörper gegen endokrines und exokrines Pankreas konnte in dem Kollektiv der Patienten mit einer CP nicht gefunden werden. Die Hypothese, dass eine Inflammation des exokrinen Pankreas eine immunologische Antwort gegen exokrines und endokrines Pankreasgewebe auslöst, konnte somit nicht bestätigt werden (Tabelle 19). Darüber hinaus ließ sich keine Spezifität für das Auftreten von CAII- und LAC-Ak (p=0,5970 bzw. 0,6911) innerhalb der einzelnen Kollektive nachweisen. Besteht neben einer exokrinen Insuffizienz bei CP der Verdacht auf eine endokrine Störung, so scheint gemäß der Interpretation dieser Studienergebnisse eine Bestimmung von ICA- und GAD-Ak als durchaus empfehlenswert, zumal deren Titerhöhe eine Aussage über eine Manifestation eines Dm1 erlaubt. Interessanterweise konnte in dieser Studie eine signifikante Korrelation zwischen der Höhe der ermittelten Titer zwischen ICA-Ak und CAII-Ak sowie CAII-Ak und LAC-Ak innerhalb aller Patienten (Gesamtkollektiv) nachgewiesen werden (p=0,0016 bzw. 0,001). Die enge Korrelation zwischen ICA-Ak und CAII-Ak, die bisher in der Literatur nicht beschrieben wurde, lässt eine gemeinsame Beteiligung autoimmuner Prozesse sowohl gegen exokrines als auch gegen endokrines Pankreas in der Pathogenese des Typ 1 Diabetes mellitus vermuten.de_DE
dc.description.abstractIn the presented thesis, the prevalence of directed autoimmune antibodies against both endocrine and exocrine pancreas tissue was examined. This was performed on patients with signs of chronic pancreatitis with and without the manifestation of diabetes mellitus and in patients with various forms of diabetes and in controls. The aim of the study was to analyze, if there is a combined prevalence of pancreatic endocrine and exocrine antibodies in patients with chronic pancreatitis, which implicates an immunological reaction against exorine and endocrine pancreatic tissue, caused by an exocrine inflammation of the pancreas. Serum samples from 48 patients with chronic pancreatitis, 48 patients with type 1 diabetes mellitus, 48 type 2 diabetes mellitus patients and 28 controls were analyzed. Indirect immunofluorescence procedures were used to test for islet cell antibodies, radio ligand-assay was used to test for glutamic acid decarboxylase and tyrosine-phosphatase IA2 antibodies, including antibodies whose corresponding antigens could be found in the islet cells in the pancreas and are used as immunological markers in autimmune diabetes. Furthermore, ELISA (enzyme-linked immunoabsorbent assay) was used to search for carbonic anhydrase II antibodies and lactoferrin antibodies. Both antibodies are immunological markers for exocrine autoimmunity. The results of the analyses are as follows: A combined prevalence concerning the analysed antibodies against endocrine and exocrine pancreas could not be found in the collective of patients with chronic pancreatitis. The hypothesis, that the inflammation of the exocrine pancreas could induce an immunological reaction against exocrine and endocrine pancreas could not be confirmed (chart 19). Furthermore, there is no evidence for the specifity of the prevalence of carbonic anhydrase II antibodies and lactoferrin antibodies within the separate collectives. If, in addition to exocrine insufficiency in chronic pancreatitis, there is a suspected endocrine dysfunction, the determination of islet cell antibodies and glutamic acid decarboxylase antibodies appears to be advisable according to the resulting interpretation of this study since manifestation of type 1 diabetes mellitus is predicted. Interestingly, in this study a significant correlation between the level of the titers of islet cell antibodies and carbonic anhydrase II antibodies as well as carbonic anhydrase II antibodies and lactoferrin antibodies within the combined collectives was determined (p=0,0016 and 0,001, respectively). The close correlation between islet cell antibodies and carbonic anhydrase antibodies, not yet described in literature, leads to the suspicion a participation of autoimmune process against both exocrine and endocrine pancreas at least in the pathogenesis of type 1 diabetes mellitus.en
