dc.description.abstractDie medikamentinduzierte Thrombozytopenie durch Abciximab, Tirofiban und/oder Eptifibatid wird in der klinischen Beobachtung, bei zunehmendem Einsatz dieser Medikamente in der interventionellen Kardiologie, in unterschiedlichem Maße gesehen. Am häufigsten tritt sie unter Abciximab auf. Die Ursache ist bis heute nicht eindeutig zu belegen. Zwei Ätiologien werden vermutet: ein immunologischer Pathomechanismus durch Antikörper vermittelt und eine nicht-immunologische Ursache durch das Medikament selbst. Diese klinisch-experimentelle Arbeit befasst sich mit der Antikörpervermittelten Thrombozytopenie. Dazu untersuchten wir die verschiedenen Kollektive auf Antikörper hin. Hierbei ließen sich bei Patienten unter einer Abciximab-/Tirofiban-/Eptifibatidtherapie, bei Patienten mit einer Herzkatheteruntersuchung und bei dem gesunden Spenderkollektiv medikament-abhängige Antikörper mit verschiedensten Untersuchungsmethoden (ELISA, Durchflusszytometer, Immunpräzipitation, MAIPA) nachweisen. Präformierte Antikörper konnten gefunden werden und werden als mögliche Ursache für die Thrombozytopenie angenommen. Erstaunlicherweise spiegelt sich die Häufigkeitsverteilung der Antikörperstärke im gesunden Kollektiv ebenfalls im Patientenkollektiv wieder (40% vs. 45%). Die Höhe des Antikörpertiters scheint dabei mit dem Ausmaß einer Thrombozytopenie zu korrelieren. Die Prävalenz der präformierten Antikörper im Spenderkollektiv gegen Tirofiban und Eptifibatid war beträchtlich niedriger (1% und 3%). Die nachgewiesenen Antikörper sind überwiegend Immunglobuline der Subklasse IgG. Die Bindungsepitope der Antikörper sind sehr heterogen und das Bindungsverhalten der Antikörper ist jeweils bei Patienten/Spendern unabhängig vom GPIIb/IIIa-Polymorphismus und dem Aktivierungszustand des GPIIb/IIIa-Komplex. In dieser Studie konnten keine Kreuzreaktionen zwischen den einzelnen Antikörpern gefunden werden. Dies ist von besonderer Wichtigkeit für eine eventuelle Therapieumstellung nach aufgetretener Nebenwirkung. Nach der ersten Medikamentapplikation scheint eine Boosterung mit Erhöhung des Antikörpertiters stattzufinden, so dass eine erneute Applikation mit dem gleichen Medikament zu einer ausgeprägteren und/oder beschleunigt auftretenden unerwünschten Nebenwirkung führen kann. Die beschriebenen Ergebnisse haben sowohl Konsequenz für die Therapie mit einem GPIIb/IIIa-Antagonisten als auch für die Nachbeobachtung. Um eine Thrombozytopenie als lebensbedrohliche Nebenwirkung zu vermeiden, sollte u.a. ggf. eine Antikörpertestung im Vorfeld durchgeführt werden. Auch muss an eine falsch positive Serologie-Testung bei AITP (Autoimmunthrombozytopenie) -Verdacht nach Abcixmab-Gabe gedacht werden.de_DE
dc.description.abstractPlatelets play a central role in the patholgenesis of coronary artery disease and acute myocardial infarction. The drug-dependent acute thrombocytopenia by abciximab, tirofiban and/or eptifibatid has been observed in different tests. Thrombocytopenia is most frequently after receiving the drug abciximab. The mechanism how the drug causes thrombocytopenia is not yet known. There are two speculations: an immunologic one, caused by drug-dependent antibodies, and an non-immunologic one by the drug itself. Sera from healthy blood donors, sera from patients treated with abciximab, tirofiban or eptifibatid and patients with intracardial catheter were screened in this study and we found drug-dependent antibodies. We used different test methods to determine the antibodys (ELISA, flowcytometrie, immunpräziptitation assays, MAIPA). We found in our patients preformed antibodies which could be seen as a possible cause of thrombocytopenia. We concluded that in some patients thrombocytopenia, followed by the injection of abciximab, tirofiban or eptifibatid, is induced by drug-dependent antibodies that cause immuologic findings similar to autoimmune thrombocytopenia. In our study, the frequency of antibodies concentration did not differ in both populations (33 patients who recieved abciximab and 100 healthy donors) (45% vs. 40%). The titer of abciximab drug-dependent antibodies was significantly higher in patient who had a decrease of their platelet count above 50% compared to patients with stable platelet count. The prevalence of performed drug-dependent antibodies against tirofiban and eptifibatid was considerably lower (3/100 and 1/100). The most of the scientifically proven antibodies belonged to the immunglobuline class IgG. Analysis of sera with a panel of mabs and stable transfectans demonstrate that epitopes of drug-dependent antibodies against different GPIIb/IIIa antagonist are heterogenous. The binding of the antibodies is independent of the GPIIb/IIIa-polymorphism and the activity of the GPIIb/IIIa-complex. Cross reactivity between the antibodies could not been establish in this study. One consequence is that we could shift the drug without any unsiderable effect. These antibodies are mostly performed but may occasionally be induced after preceding exposure. The second drug-exposition leads to boost the antibodies with an acceleration of the side-effect. These results are very important for the treatment with GPIIb/IIIa-antagonist. To avoid the life-threatening side effect with developing thrombocytopenia it can be important to analyse the sera of performed antibodies before the treatment. Furthermore it is nessesary to know, that the drug exposition with abcixmab can show a wrong positve result for autoimmunthrombocytopenia.en
