dc.description.abstractBei der systemischen Inflammation kommt es zu einer Aktivierung des Komplement- und Kontaktsystems, der Gerinnungskaskade und der Leukozyten. Dies ist mit einem Multiorganversagen assoziiert und trägt zu der hohen Mortalität der schweren Sepsis und dem septischen Schock bei [Davis et al. 2008, Zeerleder et al. 2003]. C1-INH inaktiviert Proteasen des Komplementsystems (C1r, C1s und MASP2), des Kontaktsystems (FXII, Kallikrein) und der intrinsischen Gerinnung (FXI). C1-INH wird bei der systemischen Inflammation proteolytisch inaktiviert. Die Zunahme von inaktiviertem C1-Esterase-Inhibitor korreliert mit der Mortalität der Patienten mit schwerer Sepsis und septischem Schock [Zeerleder et al. 1999]. C1-INH spielt bei der Regulation der vaskulären Permeabilität und der Inhibition der Inflammation eine wichtige Rolle. In zahlreichen tierexperimentellen Modellen mit systemischer Inflammation und Mikrozirkulationsstörung reduziert eine C1-INH Substitution die Mortalität. Die therapeutische Effektivität von C1-INH besteht in einer Suppression der Bildung von Chemoattractants, vasoaktiven Mediatoren und Aktivatoren der neutrophilen Granulozyten der plasmatischen Kaskadensysteme. Die von der Proteaseaktivität unabhängigen anti-inflammatorischen Eigenschaften beruhen auf der Inhibition der inflammatorischen Aktivität von Endotoxin, Verstärkung der Phagozytose und die Inhibition des Rollens der Leukozyten und die Transmigration durch das vaskuläre Endothel [Davis et al. 2010]. Die Leukozytenadhäsion an das mikrovaskuläre Endothel ist ein entscheidender Schritt in der Genese der Mikrozirkulationsstörung, welche als Motor der Sepsis angesehen wird [Ince 2005]. Die immuntherapeutische Toxizität von Interleukin-2 kann eine systemische Inflammation mit dem klinischen Bild eines capillary-leak-syndrom (CLS) induzieren. Die pathophysiologischen Prozesse sind denen einer Sepsis sehr ähnlich [Hack et al. 1993]. In dieser Arbeit wurde ein Sepsis-Modell durch rekombinates Interleukin-2 etabliert und eine therapeutische Beeinflussung der Mikrozirkulationsstörung durch C1-INH mittels Intravitalmikroskopie postkapillärer Venolen am Mesenterium der Ratte untersucht. Eine Substitution von C1-INH inhibiert die Mikrozirkulationsstörung einer systemischen Inflammation in dem tierexperimentellen rIL2 induzierten Sepsismodell. Die Daten verdeutlichen die potenten anti-inflammatorischen Eigenschaften von C1-INH. Die Inhibition der Entzündungskaskaden, Mediatoren und Leukozytenaktivierung beruht auf der proteaseinhibitorischen Aktivität und einer Vielzahl davon unabhängiger Mechanismen. Durch die anti-inflammatorischen Eigenschaften und das überzeugende Sicherheitsprofil ist die adjunktive C1-INH Substitution bei der systemischen Inflammation ein interessanter therapeutischer Ansatz. Kontrollierte Multicenter-Studien müssen die therapeutische Effizienz von C1-INH weiter evaluieren und darüber hinaus biologische Marker der Aktivität der plasmatischen Kaskadensysteme und der Leukozyten identifizieren, um Patienten mit einem tatsächlichen Nutzen einer C1-INH Therapie selektionieren zu können.de_DE
dc.description.abstractDuring systemic inflammation the complement system, the contact system, the coagulation cascade and leukocytes are activated. Activation of these plasmatic cascades und leukocytes contributes to the development of multiple organ failure and the high mortality rate of severe sepsis and septic shock [Davis et al. 2008, Zeerleder et al. 2003]. C1-INH is an main inhibitor of the classical pathway of the complement system (C1s and C1r, MASP2), of the contact activation system (factor XIIa and kallikrein) and of the intrinsic pathway of coagulation (factor XIa). During systemic inflammation, C1-INH is proteolytically inactivated. The increase of inactivated C1-INH in plasma correlates positively with mortality in septic patients. C1-INH substitution has been shown to reduce the mortality in experimental animals with severe sepsis or septic shock [Zeerleder et al. 1999]. C1-INH plays important roles in the regulation of vascular permeability and in the suppression of inflammation. The effectiveness of C1-INH attributes to its protease inhibition activity dependent properties to attenuate the generation of chemoattractants and vasoactive mediators as well as activators of neutrophils. C1-INH mediated activities that do not depend on protease inhibition contributes suppression of the activity of gram negative bacterial endotoxin, enhancement of phagocytosis, and suppression of leukocyte rolling and transmigration of leukocytes across the endothelium to sites of inflammation [Davis et al. 2010]. The leukocyte adhesion to the microvascular endothelium is a critical step in the genesis of microcirculatory disturbance, which is regarded as the "motor" of sepsis [Ince 2005]. Immunotherapy with Interleukin-2 can cause a systemic inflammation called capillary-leak-syndrom (CLS). The pathophysiological mechanisms are similar to those invoke during sepsis [Hack et al. 1993]. In this work an animal sepsis model was established by recombinant IL-2 to investigate therapeutic influence on microcirculatory disturbance by C1-INH examined by intravital microscopy in postcapillary venules of rat mesentery.C1-INH substitution inhibits microcirculatory disturbance during rIL2 induced experimental systemic inflammation. The data illustrate that C1-INH has potent anti-inflammatory capacity. Protection from inflammatory diseases, sepsis and septic shock very likely is a result of a variety of mechanisms that may or may not depend on protease inhibition via covalent bonding. Owing to its potent anti-inflammatory properties and its convincing safety profile C1-INH is an attractive therapeutic protein and may be an adjunctive in the treatment of Sepsis and SIRS. Controlled multi-centre studies must be conducted to prove the beneficial effects of C1-INH and detect activation markers of the plasmatic cascade system and leukocyte activation to identify which patients will truly benefit.en
