dc.description.abstractHintergrund: Durch Verbesserung der Überlebenswahrscheinlichkeit nach Herztransplantation (HTX) im Kindesalter bekommen Langzeitkomplikationen eine zunehmende Bedeutung. Die posttransplantationslymphoproliferative Erkrankung (PTLD) als potentiell lebensbedrohliche Erkrankung entsteht auf dem Boden der lebenslangen Immunsuppression und ist bei Kindern in den meisten Fällen mit dem Epstein- Barr- Virus (EBV) assoziiert. Als weitere Risikofaktoren für die Entwicklung einer PTLD werden junges Patientenalter bei Transplantation, Infektionen mit dem Cytomegalievirus (CMV), Abstoßungen und verschiedene immunsuppressive Medikamente diskutiert. Das Ziel dieser Arbeit war, die Häufigkeit, Lokalisation, Pathologie, EBV- Assoziation, Therapie der PTLD sowie das Outcome der Patienten zu definieren und orientierend Risikofaktoren für die Entwicklung einer PTLD in unserer Patientengruppe zu identifizieren.Methodik: Am Giessener Kinderherzzentrum wurden 133 Patienten nach HTX im Kindesalter, die nach HTX mindestens drei Monate überlebten, retrospektiv über einen Zeitraum von 26 Jahren (1988-2014) untersucht. Die Charakteristika der PTLD- Patienten wurden mit denen der Patienten ohne PTLD verglichen. Ausschlusskriterien waren Alter bei HTX über 18 Jahre, keine durchgehende Betreuung im Kinder- herzzentrum, Tod innerhalb der ersten drei Monate nach HTX oder ein Follow- up von weniger als drei Monaten.Ergebnisse: 16% der eingeschlossenen Patienten (21/133) entwickelten im Median nach 7,2 Jahren (0,5- 14,9 Jahren) im Alter von 8,3 Jahren (1,6- 25,8 Jahren) eine PTLD. PTLD- Patienten waren zum Zeitpunkt der HTX tendenziell (nicht signifikant) jünger als die Vergleichspatienten und zeigten häufiger eine EBV- und CMV- Seronegativität. Die mittlere Anzahl therapierter Abstoßungen und der Anteil der mit Antithymozytenglobulin (ATG) behandelten Patienten waren in der Gruppe der PTLD- Patienten nicht signifikant höher als in der Vergleichsgruppe.Als häufigste Lokalisation der PTLD stellte sich der Kopf- Hals- Bereich (ohne ZNS) dar, gefolgt vom Abdomen. Histologisch fanden sich zu jeweils einem Drittel (7/21) ein monomorphes diffus- großzelliges B- Zell- Lymphom (DLBCL) und eine polymorphe PTLD. Drei Patienten wiesen ein Hodgkin- Lymphom bzw. eine Hodgkin- like PTLD auf und jeweils zwei Patienten eine monomorphe B- Zell- PTLD vom Burkitt- Typ sowie eine monomorphe T- Zell- PTLD. EBV konnte bei 75% der Tumore mittels EBER- 1- in situ- Hybridisierung nachgewiesen werden, wobei ein EBV- Nachweis signifikant mitfrüherem Auftreten der PTLD nach HTX korrelierte. PTLD- Patienten hatten signifikant häufiger einen positiven EBV- Genom- Nachweis im Blut als Kontrollpatienten. Zum Zeitpunkt der PTLD- Diagnose lag bei 77% der Patienten eine hohe EBV- Viruslast vor. 38% der PTLD- Patienten entwickelten eine rekurrierende PTLD, bei der es sich in fünf von acht Fällen nicht um Rezidive, sondern um Zweittumore handelte. In der Gruppe der Rezidive und Zweittumore stellten DLBCL den häufigsten histologischen Subtyp dar (4/8), gefolgt von der Hodgkin- like PTLD (2/8). Neben der Reduktion der Immun- suppression umfasste die Therapie je nach PTLD- Subtyp, Lokalisation, Stadium und Ansprechen Rituximab, Chemotherapie, eine chirurgische Resektion, Bestrahlung und/ oder eine Immuntherapie mit EBV- spezifischen cytotoxischen T- Zellen (CTL). In der PTLD- Gruppe überlebten bis zum Ende des Studienzeitraums 71% der Patienten (15/21), in der Gruppe der Patienten ohne PTLD 93% (104/12).Schlussfolgerung: Bei der PTLD handelt es sich um eine histologisch heterogene Erkrankung. Das Langzeitüberleben nach PTLD ist trotz guter Therapieerfolge erheblich beeinträchtigt. Der zukünftige Fokus sollte neben der Entwicklung neuer Therapieoptionen auf der Prävention der Erkrankung und der genauen Definition von Risikofaktoren liegen.de_DE
dc.description.abstractBackground: Because of the increasing long- term survival after pediatric heart transplantation, long- term complications due to the lifetime immunosuppression become more relevant. Posttransplant lymphoproliferative disease (PTLD) is a potentially life- threatening event, which can mostly be associated with the Epstein- Barr virus (EBV), particularly in the childhood. Further clinical risk factors, such as a young age at the time of transplantation, infections with CMV, rejections and the type of immunosuppression are discussed in literature. The aim of the study was to define the incidence, clinical manifestation, histology, association to EBV, treatment and outcome and to evaluate potential risk factors for the development of PTLD in our patients.Methods: At the Pediatric Heart Center Giessen 21 patients suffering from PTLD were retrospectively reviewed over a study- period of 26 years (1988-2014) and compared to 112 healthy heart- transplanted children. Criterias for exclusion were ages over 18 years at time of transplantation, death after HTX within three months, no regular follow- up or minimal follow- up less than three months.Results: PTLD was diagnosed in 16% (21/133) of the included patients at a median age of 8.3 years (1.6- 25.8 y/o) after a median post- transplant period of 7.2 years (0.5-14.9 y/o). Concerning the PTLD- patients, there was a trend (not significant) to younger age at transplantation and to a higher frequency of seronegativity of EBV and CMV compared to the patients without PTLD. Mean rejection episodes and treatment with ATG were insignificantly higher in the PTLD- patient group.Most common sites of PTLD were head and neck (without CNS), followed by the abdomen. Histology was most frequently monomorphic DLBCL (7/21) and polymorphic PTLD (7/21). Three patients showed a Hodgkin- like PTLD, two patients a monomorphic Burkitt- lymphoma and two more patients a monomorphic T- cell- PTLD. 75% of the tumours were EBV- positive by EBER-1 in- situ hybridization, which was significantly correlated with earlier occurrence of PTLD. Within the PTLD- patient group, there was a significant higher frequency of elevated EBV- viral load in the blood than in the comparison group. At the time of the first diagnosis 77% of the PTLD- patients showed a high viral load.Recurrent PTLD occurred in 38% of the cases, but more often (5/8) as second malignancy than as relapse. Within the recurrent PTLD, DLBCL was diagnosed most frequently in four cases, followed by Hodgkin- like PTLD in two cases.Besides reduction of immunosuppression, which was performed in all patients, therapy consisted of rituximab, chemotherapy, surgery, radiation and/or EBV- specific cytotoxic T- cell therapy (CTL). Survival over the study period was 71% (15/21) in the PTLD- patient group and 93% (104/112) in the comparison group without PTLD.Conclusion: PTLD is a histologically heterogeneous disease. The long- term survival after pediatric heart transplantation can be seriously impaired by PTLD, despite the appropriate therapy. Besides the development of new treatment options, further efforts are needed to define risk factors exactly and to prevent PTLD.en
