dc.description.abstractDie Knochenmarktransplantation (KMT) ist ein kuratives Therapieverfahren, das heute bei vielen hämatologisch-onkologischen Erkrankungen eingesetzt wird. Nach allogener KMT kann es allerdings zur klinischen Komplikation der graft-versus-host-disease (GvHD) kommen, die aufgrund der Histoinkompatibilität von Donor und Rezipient auftritt. Hohe Morbidität und Mortalität bedingt durch eine hCMV-Infektion kennzeichnen zudem das Krankheitsbild der immunsupprimierten Patienten nach allogener KMT. Die Untersuchungen der vorliegenden Arbeit wurden in einem murinen KMT Modell durchgeführt, bei dem die murine Cytomegalovirus (mCMV) Infektion ohne Transfer von Knochenmarkzellen (KMZ) letal verlaufen würde. Um die klinische Problematik der allogenen KMT zu studieren, wurden Mäuse der Stämme BALB/cJ (Kd, Dd, Ld) und die Ld Deletionsmutante BALB/c-H-2dm2 (Kd, Dd) - beide des Haplotyps H-2d verwendet, die sich in der Expression des MHC-Klasse-I-Moleküls Ld unterscheiden. Die beiden Mausstämme wurden wechselseitig als Donoren oder Rezipienten eingesetzt. Vorarbeiten hatten gezeigt, dass die Tiere nach host-versus-graft-KMT (HvG-KMT; BALB/cJ & #61664; BALB/c-H-2dm2) und konkurrenter mCMV-Infektion überleben, während die Tiere nach GvH-KMT (BALB/c-H-2dm2 & #61664; BALB/cJ) und konkurrenter mCMV-Infektion sterben. Warum die Tiere unter diesen Bedingungen sterben, war die Hauptfragestellung dieser Arbeit.Die Ergebnisse weisen auf eine unterschiedliche Kontrolle der mCMV-Infektion nach HvG- und GvH-KMT hin. Nach GvH-KMT zeigten sich in der Leber viele infizierte Zellen. Die Infektion wurde trotz hoher Infiltration durch CD3 Zellen nicht kontrolliert. Die CD3 Zellen und die infizierten Zellen waren diffus verteilt, während sich nach HvG-KMT eine Fokusbildung zeigte, was auf eine zielgerichtete Infiltration und somit Protektion deutete. Zusätzlich wurden nach GvH-KMT im Vergleich zur HvG-KMT und syngenen KMT nur wenige mCMV-peptidspezifische CD8-T-Zellen nachgewiesen. Stattdessen kam es zur Aktivierung von alloreaktiven CD8 T-Zellen.Nach GvH-KMT überlebten alle Tiere, sofern die Infektion mit der Immunevasin- Deletionsmutante mCMV-& #916;vRAP erfolgte. Da auf mCMV-& #916;vRAP infizierten Zellen die Peptidpräsentation im Vergleich zu mCMV-WT infizierten Zellen erhöht ist, ist die fehlende Protektion nach GvH-KMT und mCMV-WT Infektion vermutlich auf eine unzureichende Peptidpräsentation zurückzuführen. Dies hat wiederum ein geringes CD8 T-Zell-Priming zur Folge. Nach GvH-KMT und Infektion mit mCMV-& #916;vRAP scheint die Menge präsentierten Peptids wieder auszureichen, um die mCMV-Infektion zu kontrollieren.de_DE
dc.description.abstractCMV infection plays an important role in immunocompromised patients, especially in the context of allogeneic bone marrow transplantation (BMT). Histoincompatibility between donor and recipient is associated with the risk of graft-versus-host-disease (GvHD).We established a murine model of cytomegalovirus (CMV) infection after allogeneic BMT. Specifically, we used two mouse strains (haplotype H-2d) with a single MHC class-I disparity as donors or recipients: the BALB/c mouse (Kd, Dd, Ld) and the Ld-deletion mutant BALB/c-H-2dm2 (Kd, Dd). Whereas most of the animals survive host-versus-graft-BMT (HvG-BMT; BALB/c  BALB/c-H-2dm2) and concurrent mCMV infection, GvH-BMT (BALB/c-H-2dm2  BALB/c) is lethal. The clinically important question, why the GvH-BMT is associated with such a dramatic outcome was the subject of this work.The control of mCMV-infection was impeded after GvH-BMT, associated with a high number of infected cells in the organs. Besides the high number of infected cells, there were also a high number of CD3 cells detectable in the liver. Nevertheless, the CD3 cells were not able to control the infection. After HvG-BMT, infected cells were organized in foci surrounded by CD3 cells, a histologic manifestation of control of mCMV-infection, whereas after GvH-BMT infected cells as well as CD3 cells were distributed randomly. CD3 cells proved to be mCMV peptide-specific CD8 T cells as well as alloreactive cells. Remarkably, the number of mCMV peptide-specific CD8 T cells was very low after GvH-BMT compared to HvG-BMT and syngeneic BMT. In contrast to mCMV-WT infection, the animals survive GvH-BMT and infection with mCMV-ΔvRAP, an immunoevasin gene deletion mutant of mCMV. After infection with mCMV-ΔvRAP, the infected cells present more peptide in comparison to cells infected with mCMV-WT. Therefore, the deficient control of mCMV-WT infection is probably based on an insufficient peptide presentation. After GvH-BMT and infection with mCMV-ΔvRAP, peptide presentation is apperently sufficient to support CD8 T cell priming and to prevent the lethal course of CMV disease.en
