dc.description.abstractDas Immunsystem, bestehend aus angeborenen und adaptiven Komponenten, stellt eine wichtige Schutzfunktion des Organismus vor Infektionen dar. Traumata unterschiedlicher Genese können eine massive systemische Inflammation induzieren und zu lebensbedrohlichem Multi- Organversagen führen. Im Rahmen dieser Arbeit wurde in einem ersten Überblick die Immunmodulation in der Lunge von Mäusen nach stumpfen Skelettmuskeltrauma auf die Gastrocnemiusmuskulatur untersucht. Die signifikante Rekrutierung neutrophiler Granulozyten in die Lunge nach stumpfem Trauma der Gastrocnemiusmuskulatur verdeutlicht den massiven Einfluss eines traumatischen Insultes auf das Immunsystem. Das Maximum der Granulozyteninfiltration in die Lunge und das Blut von Mäusen des Wildtyps wurde drei Stunden nach Skelettmuskeltrauma erreicht, wobei die Zahl der Granulozyten nach Trauma als normalisierter Wert des Medians der Kontrollgruppe von 100% in der Lunge auf 232,16% und im Blut auf 176,95% anstieg. Auch bei MyD88-/-- Mäusen wurde ein Anstieg der Granulozyten im Median in den Lungen der traumatisierten Tiere um das 2,8 fache und im Blut um das 2,5 fache gemessen und bei TLR4-/--Mäusen in der Lunge um das 2,9 fache. Zeitgleich konnte eine signifikante Erhöhung des Medians der Serumkonzentration an S100A8/ S100A9-Kalzium-Proteinen von 427 ng/ml auf 1396,5 ng/pg gemessen werden. Vermehrt unreife Granulozyten mit geringer Expression von CD11b Protein auf der Zelloberfläche wanderten drei Stunden nach Trauma in die Lunge ein, gemessen mit der mean fluorescence intenensity in der Narkosekontrollgruppe von 6165,25 erniedrigt bei Mäusen nach Skelettmuskeltrauma auf 5634,95, jedoch ohne gleichzeitig ROS Produktion oder histopathologische Veränderungen der Lunge hervorzurufen. Nach sekundärer Pseudomonas- Infektion wurde eine Einwanderung neutrophiler Granulozyten pulmonal und hämatologisch sowohl mit als auch ohne Traumainduktion unter Zunahme der ROS- Produktion beobachtet. In der Lunge erhöhte sich die Zahl der Granulozyten drei Stunden nach Trauma und nachfolgender Pseudomonasinfektion von 100% der Narkosekontrollgruppe ohne Peudomonasinfektion auf 700,13% in der infizierten Narkosekontrollgruppe und auf 860,75% nach Traumatisierung, vier Tage nach Trauma und Infektion von Pseudomonas- Bakterien ohne Trauma auf 594,23% und nach Weichteiltraumainduktion auf 1001,17%. Auch Transfer von Serum in naive Tiere von Tieren der Narkosekontrolle und Tieren drei Stunden nach Trauma führte zu einer signifikatnen Infiltration von Granulozyten in die Lunge der naiven Tiere von einem normalisierten Wert 100% der Tiere behandelt mit Serum der Narkosekontrolle auf 160,94 % der Mäuse behandelt mit Serum der Traumagruppe. Die Inflammation befand sich drei Stunden nach Trauma in einer prädominanten granulozytären Phase mit einer unveränderten IL- 6- Konzentration und Abfall der Konzentrationen von IFN- g von 6517,1 pg/ml in der Narkosekontrollgruppe auf 5066,9 pg/ml in der Traumagruppe und IL- 10 von 218,2 pg/ml in der Narkosekontrollgruppe auf 178,6 pg/ml in der Traumagruppe in der Lungenkultur nach Restimmulation mit UV- inaktivierten Pseudomonas aeruginosa Bakterien. Diese Phase ist geprägt durch massive Akkumulation neutrophiler Granulozyten, unterstützt durch die erhöhten Werte der auf Granulozyten chemotaktisch wirksamen Chemokine KC und MIP- 2. Nach Restimmulation der Lungenkultur mit Pseudomonas- Bakterien stieg die KC-Konzentration von 9342,33 pg/ml der Narkosekontrollgruppe auf 12152,3 pg/ml der Traumagruppe und die Konzentration von MIP- 2 von 1396,5 pg/ml der Narkosekontrollgruppe auf 37721,3 pg/ml der Traumagruppe. Der statistische Anteil der Zellpopulationen an Alveolar Makrophagen, Monozyten, Neutralen Killer- Zellen, Neutralen Killer T- Zellen, T- Zellen und Dendritischen Zellen blieb zum Zeitpunkt drei Stunden nach Trauma dagegen konstant. Eine Infektion der traumatisierten Tiere mit Pseudomonaden ergab keine gestörte Granulozytenrekrutierung. Das angewendete Skelettmuskel- Trauma führte zu einer Inflammation durch Granulozyteneinstrom, jedoch ohne Organschädigung zu verursachen wie bei schweren Traumata beschrieben. Die Entzündungsreaktion in der Lunge wird durch lösliche Faktoren im Blut vermittelt. Die Trauma- induzierte Rekrutierung von Granulozyten in die Lunge könnte einen prädisponierenden Faktor für die Entwicklung eines acute respiratory distress syndrome (ARDS) darstellen, das mit einer erhöhten Mortalitätsrate assoziiert ist.de_DE
dc.description.abstractTraumata induce severe systemic inflammation that may cause life- threatening multiorgan failure.The study addresses the immunological effect in the lung of mice after soft tissue trauma of the gastrocnemic muscle. Significant recruitment of granulocytes from periphery or bone marrow into the lung after soft tissue trauma in the gastrocnemic muscle of the mice demonstrates the massive systemic influence of traumata. The infiltration rate of neutrophils into lung and blood of wild type animals reached its maximum three hours past trauma induction, whereat the number of granulocytes increased from 100% in the control group to 232% in lung and 177% in blood for the traumatized mice, as given by the normalized merit of the median. Also for MyD88-/-- mice, the median of the number of granulocytes in the lung increased 2.8 times after trauma and 2.5 times in blood; for TLR4-/-- mice after trauma the granulocytes increased 2.9 times in the lung. The effect was accompanied by a significant increase of S100A8/ S100A9 concentration in blood serum from 427 ng/ml to 1397 ng/pg. Less mature granulocytes, detectable by their low abundance of CD11b surface protein, also immigrated into the lung as was measured by mean fluorescence intensity. Three hours after soft tissue trauma the concentration had increased from 6165 ng/pg in control mice to 5635 ng/pg for the trauma group, without causing ROS production or histopathological changes in the lung. Secondary infection of mice with P. aeruginosa after trauma as well as of control mice led to an extended infiltration of neutrophils into lung and blood. In this case, however, the increase of ganulocytes was accompanied by measurable ROS production. The number of granulocyte in the lung increased within three hours after trauma and infection with pseudomonas bacteria from 100% for the control group without infection to 700% in the control group with infection, and to 861% after trauma and infection. Four days after trauma and infection an increase of 594% was found for the control group and 1001% for the trauma group. Three hours after trauma there was a pre-dominant granulocytary state of inflammation without changes in IL- 6- cytokine concentration and a decrease in the concentration of IFN- g from 6517 pg/ml in control group to 5067 pg/ml after trauma and a decrease in the concentration of IL- 10 from 218,2 pg/ml in control group to 178,6 pg/ml after trauma in lung culture after restimmulation with UV- inactivated Pseudomonas aeruginosa bacteria. Transfer of serum from mice of thecontrol group and from mice of the trauma group three hours after trauma to naïve mice induced significant infiltration of granulocytes into the lung of the naïve mice. The normalized merit raised from 100% for the control group to 161% in naïve mice after serum transfer from traumatized mice. This state of inflammation is characterized by massive accumulation of neutrophils challenged by increased concentration of chemokines KC and MIP-2. After restimmulation with UV-inactivated pseudomonas bacteria, in lung culture the concentration of KC increased from 9342 pg/ml for the control group to 12152 pg/ml after trauma and the concentration of MIP- 2 increased from 1396 pg/ml for the control group to 37721 pg/ml after trauma. Three hours after soft tissue damage, the statistical percentage of alveolar macrophages, monocytes, neutral killer cells, neutral killer T- cells, T- cells and dendritic cells did not change in the cell population.The infection of traumatized mice with pseudomonas did not cause any defective infiltration of granulocytes. There were also no increase in chemokine production or any histopathological defect of the lung. The muscle trauma resulted in inflammation as well as infiltration of granulocytes without causing organ damage, known to occur for severe traumata. The inflammation of the lung is mediated by soluble components in blood.The trauma- induced recruitment of granulocytes into the lung may predispose the organism to the development of acute respiratory distress syndrome (ARDS) that is associated with enhanced mortality.en
