dc.description.abstractDie besondere Bedeutung des atrialen natriuretischen Peptids bei Patienten mit koronarer Herzrerkrankung liegt in seiner Fähigkeit, sowohl den funktionellen Störungen im Sinne einer linksventrikulären Funktionseinschränkung entgegen zu wirken, als auch strukturellen Veränderungen im Sinne eines vaskulären Remodellings. Wir untersuchten die Plasmakonzentrationen an atrialem natriuretischen Peptid bei Patienten mit koronarer Herzerkrankung im Hinblick auf die Frage, in wie weit die Höhe der Plasma-ANP-Spiegel auf die Schwere der koronaren Schädigung schließen läßt und in wie weit die Höhe der ANP-Spiegel eine Einschränkung der linksventrikulären Funktion widerspiegeln. Darüber hinaus untersuchten wir die Beziehung des atrialen natriuretischen Peptids zur kardialen Hämodynamik bei Herzinsuffizienz anderer Genese als der koronaren Herzerkrankung. Bei 29 Patienten mit pectanginöser Beschwerdesymptomatik wurden die ANP-Spiegel gemessen und eine Koronarangiographie durchgeführt. 7 der 29 Patienten bildeten eine weitere Studiengruppe und unterzogen sich einer spiroergometrischen Untersuchung sowie einer Blutgasanalyse vor und nach Belastung. Bei 16 Patienten mit komplexen Herzerkrankungen wurde eine Rechtsherzkatheteruntersuchung mit Bestimmung von ANP an multiplen Abnahmeorten durchgeführt. Unsere Ergebnisse bestätigen, dass die Höhe der ANP-Werte auf die Schwere der linksventrikulären Funktionseinschränkung schließen lassen (ANP vs. lvfdp r = 0,65; p = 0,002 und ANP vs. lvedp r = 0,60; p = 0,01). Darüber hinaus wird hier erstmals beschrieben, dass die Schwere der koronaren Herzerkrankung mit der Höhe des ANP-Spiegels korreliert (Koroscore vs. ANP r = 0,61; p= 0,0005). Die Belastungsuntersuchungen bestätigen, dass bei körperlicher Belastung der ANP-Spiegel ansteigt ( ANP-Ruhe vs. ANP-Belastung r = 0,95; p < 0,001). Darüber hinaus konnte gezeigt werden, dass der Anstieg um so größer ist, je höher der ANP-Ruhewert ist (Delta-ANP vs. ANP-Ruhe r = 0,82; p = 0,02). Es konnte erstmals ein enger Zusammenhang zwischen Blutgasen und dem ANP-Verhalten beschrieben werden. Je mehr das Verhältnis von ANP in Ruhe zu ANP unter Belastungsbedingungen anstieg, desto mehr steigen die Sauerstoffpartialdrucke bei Belastung (ANP-ratio vs. PO2max r = 0,97; p = 0,0004). Waren hohe ANP-Werte in Ruhe zu messen, so war die Differenz zwischen dem Sauerstoffpartialdruck in Ruhe und bei Belastung geringer, als bei niedrigeren ANP-Ruhe-Werten (Delta-PO2 vs. ANP-Ruhe r = 0,82; p = 0,02). Die hämodynamischen Daten im Rahmen der Rechtsherzkatheteruntersuchung bestätigen den kausalen Zusammenhang zwischen dem pulmonal vaskulären Widerstand, dem pulmonal arteriellen Druck, dem rechtsatrialen Druck und dem linksventrikulären enddiastolischen Druck (RAP vs. PVR r = 0,55; p = 0,04; PAP vs. RAP r = 0,75; p = 0,00076; PAP vs. lvedp r= 0,57; p = 0,024). Die Höhe der ANP-Spiegel korreliert mit dem linksventrikulären frühdiastolischen Druck, dem linksventrikulären enddiastolischen Druck, dem pulmonal vaskulären Druck und der Sauerstoffsättigung im rechten Vorhof ( ANP vs. lvfdp r= 0,716; p = 0,003; ANP vs. lvedp r= 0,62; p = 0,01; ANP vs. pvr r = 0,56; p = 0,02; ANP vs. SO2 RA r = 0,63; p = 0,01). Insgesamt bestätigen unsere Ergebnisse die Bedeutung des atrialen natriuretischen Peptids im Zusammenhang mit einer linksventrikulären Dysfunktion auf dem Boden einer koronaren Herzerkrankung. Die Höhe der Korrelation zwischen den ANP-Werten und dem Koroscore und die deutliche statistische Signifikanz weisen auf eine enge Verknüpfung hin, die nicht allein über die systolische linksventrikuläre Funktion erklärt werden kann. Die hämodynamischen Messungen legen nahe, dass sich die koronare Herzkrankheit auf die diastolische Funktion und folglich auf den diastolischen Druck auswirken und so letztlich den Zusammenhang zwischen Koroscore und ANP-Spiegel erklären. Die Messung von ANP-Werten unter Ruhe- und Belastungsbedingungen im Rahmen einer Spiroergometrie mit Blutgasanalyse liefert richtungsweisende Befunde in Bezug auf die objektive Belastbarkeit des Patienten und den Schweregrad seiner Herzerkrankung. Zusätzlich ergeben sich Hinweise auf die Regulation des pulmonalen Blutflusses, welche von der hypoxischen pulmonalen Vasokonstriktion der linksventrikulären Funktion und vom pulmonalem Druck beeinflußt wird.de_DE
dc.description.abstractThe special importance of atrial natriuretic peptide (ANP) in patients with coronary artery disease lies in its ability to counteract both the functional disturbance along with left ventricular dysfunction and the structural changes going ahead with vascular remodeling. We explored the plasma concentrations of atrial natriuretic peptide in patients with coronary heart disease to find out whether the level of ANP refers to the severity of the underlying coronary heart disease and how the ANP level refers to the extend of the left ventricular dysfunction. Moreover we investigated the relation between ANP and the cardiac hemodynamics in patients with chronic heart failure of other genesis than coronary heart disease. In 29 patients with symptoms of angina pectoris we measured the levels of atrial natriuretic peptide and carried out an coronary angiography. 7 out of the 29 patients constituted another group of investigation; they underwent a spirometric exercise test including an bloodgas analysis before and after the exercise test. In 16 patients with complex heart disease we carried out a right ventricular catheterization with measurement of atrial natriuretic peptide on multiple intracardial locations. Our results confirm that the levels of ANP estimate the severity of the left ventricular dysfunction ( ANP vs. lvfdp r = 0.65; p = 0.002 and ANP vs. lvedp r = 0.60; p = 0.01). Beyond this, it is the first time to describe that the severity of the underlying coronary heart disease is correlated with the levels of atrial natriuretic peptide (coroscore vs. ANP r = 0.61; p = 0.0005). The exercise tests confirm that the levels of ANP rise with the insensity of physical exercise (ANP-at rest vs. ANP-exercise r = 0.95; p < 0.0001). Moreover our results show that with higher ANP levels at rest the rise of ANP-exercise increases (Delta-ANP vs. ANPat rest r = 0.82; p = 0.02). For the first time we could describe an close relation between the levels of bloodgas and the corresponding levels of ANP. The more the ratio of ANP at rest to ANP at exercise increased, the more the levels of oxygen partial pressure increased (ANP ratio vs. PO2max r = 0.97; p = 0.0004). Measuring high levels of ANP at rest meant a smaller difference between oxygen partial pressure at rest and oxygen partial pressure with exercise, than with smaller levels of ANP at rest (Delta-PO2 vs. ANP at rest r = 0.82; p = 0.02). The hemodynamic data we investigated with right ventricular catheterization confirmed the relation between pulmonary vascular resistance, pulmonary artery pressure, right atrial pressure and left ventricular enddiastolic pressure (RAP vs. PVR r = 0.55; p = 0.04; PAP vs. RAP r = 0.75; p = 0.00076; PAP vs. lvedp r = 0.57; p = 0.024). The level of ANP correlated with left ventricular pressure at onset of diastole, the left ventricular enddiastolic pressure, the pulmonary vascular pressure and the oxygenation in right atrium (ANP vs. left ventricular pressure at onset of diastole r = 0.716; p = 0.003; ANP vs. lvedp r = 0.62; p = 0.01; ANP vs. pvr r = 0.56; p = 0.02; ANP vs. SO2 RA r = 0.63; p = 0.01). In summary, our results confirm the importance of atrial natriuretic peptides in relation to left ventricular dysfunction out of coronary artery disease. The level of correlation between ANP data and the data of coroscore and the clear statistical significance indicate a close connection, which cannot explained only by the systolic left ventricular function. The hemodynamic measurements confirm, that the coronary artery disease has an effect on diastolic function and consequently on diastolic pressure what at last explains the relation between coroscore and ANPlevels. The measuring of ANP levels at rest and at exercise during spirometry with bloodgas analysis gives guiding reference to the objective patient s fitness and the severity of the patient s heart disease. In addition, there are indications to the regulation of pulmonary bloodflow, which is influenced by hypoxic pulmonary vasoconstriction of left ventricular function and by pulmonary pressure.en
