dc.description.abstractDas kolorektale Karzinom gehört zu den drei häufigsten bösartigen Neubildungen. Bedingt das Malignom eine Obstruktion stellt dies einen Notfall dar der einer sofortigen Therapie bedarf. Die Operation ist hierbei das einzige akutinterventionelle Verfahren, das bezüglich des Malignoms einen kurativen Ansatz bietet. Der Notfalleingriff besitzt jedoch, verglichen mit dem elektiven Vorgehen, eine deutlich erhöhte Morbidität und Mortalität. Ist zudem der Tumor nicht resektabel, wird zur Wiederherstellung der Stuhlpassage häufig die Anlage eines Anus praeter notwendig. Ziel der vorliegenden Arbeit war es, die Effektivität der endoskopischen Stentimplantation zu untersuchen. Betrachtet wurden neben dem technischen Erfolg die Wiederherstellung der Stuhlpassage, womit eine elektive, einzeitige Operation ermöglicht werden sollte. Bei Patienten die aufgrund eines ausgedehnten Lokalbefundes oder bestehender Fernmetastasierung inkurabel waren, erfolgte die Stentimplantation als palliative Maßnahme. Die vorliegende Arbeit stellt mit 46 Patienten die aktuell größte monozentrische Studie mit endoskopisch/radiologischer Stentimplantation dar. Bei 93,3 % der Patienten ließ sich die Stuhlpassage wiederherstellen. Alle fünf Patienten die in kurativer Absicht mit einem Stent versorgt wurden, konnten einer elektiven, einzeitigen Operation zugeführt werden. In der Palliativsituation wurde durch die Stentimplantation in 90% der Fälle die Anlage eines Anus praeter vermieden. Im Verlauf auftretende, tumorbedingte Komplikationen waren mit nicht-operativen Verfahren wie Nd: YAG-Laser, Ballondilatation oder Anlage eines zweiten Stent mehrheitlich zu beherrschen. Die hier vorgestellten Ergebnisse erweisen die Stentimplantation als ein sicheres Verfahren mit hoher Erfolgsrate. Sie kann zur Dekompression bei der Primärdiagnostik des akuten kolorektalen Karzinoms ebenso wie zur Palliativversorgung empfohlen werden. Das Verfahren wird in sofern eingeschränkt, als es nur durch erfahrene Endoskopiker anwendbar ist. Prospektive Studien im Vergleich zum operativen Vorgehen sind wünschenswert, aufgrund der höheren Patientenakzeptanz nicht-operativer Maßnahmen jedoch nur schwer realisierbar.de_DE
dc.description.abstractThe colorectal carcinoma is one of the three most common malignant growths. Obstruction caused by the tumour is an emergency requiring instant therapy, surgery being the only acute interventional procedure of a curative nature. Emergency surgery, however, brings significantly higher rates of morbidity and mortality than an elective procedure. Also, in cases where the tumour is unresectable, colostomy is often necessary to resolve bowel obstruction. The aim of this study was to examine the effectiveness of endoscopic stent implantation. The resolution of bowel obstruction, opening the way for elective one-stage surgery was evaluated as well as technical success. Palliative patients (with extensive local findings or metastastic cancer) received stent implantation as a palliative measure. This study, covering the cases of 46 patients, is currently the largest monocentric study of endoscopic /radiological stent implantation. For 93.3% of the patients it was possible to resolve bowel obstruction. All 5 patients who were given a stent with curative intentions were able to have elective one-stage surgery. For the palliative patients the stent implantation meant that in 90% of cases colostomy could be avoided. Tumour-related complications which arose were mainly treated with non-surgical procedures such as Nd. YAG_Laser, balloon dilation or the insertion of a second stent The results presented here demonstrate that stent implantation is a safe procedure with a high success rate. It can be recommended for decompression during the initial diagnosis of colorectal carcinoma as well as for palliation. The use of the procedure is, however, limited by the fact that it can only be carried out by experienced endoscopists. Prospective studies comparing it to surgical methods would be desirable, but because of the higher patient acceptance of non-surgical methods they would be difficult to realise.en
