Der unicondyläre Gelenkflächenersatz als Therapiekonzept der Varus- / Valgusgonarthrose

Abstract

298 (w: 226 / m: 72) mit einer unicondylären Schlittenprothese versorgte Patienten aus dem Implantationszeitraum 1975 bis 1995, konnten im Rahmen dieser retrospektiven Studie in der orthopädischen Klinik am Ev. Fachkrankenhaus in Ratingen nachuntersucht werden. Die Gesamtzahl der bei diesem Patientengut implantierten Prothesen betrug 341. In 94 % der Fälle fand eine Versorgung des medialen, in 6 % des lateralen Kompartiments statt. Zur Scoreberechnung wurden 308 Prothesen herangezogen. Das Durchschnittsalter zum Zeitpunkt der Implantation betrug 65,4 Jahre, zum Zeitpunkt der Nachuntersuchung 76,8 Jahre. Die durchschnittliche Prothesenstandzeit belief sich auf 5,6 Jahre. Die Ergebnisse gemäß des Knee-Society-Scores stellten sich wie folgt dar: Präoperativ 57,67 Pkt. Postoperativ 160,39 Pkt. Der Kniescore ergab einen postoperativen Wert von 88,9 Pkt., der Funktionsscore von 71,49 Pkt. Die Überlebensraten, berechnet nach der KAPLAN-MEIER-Methode, äußern sich in einem 5 Jahreswert von 96,1 %, einem 10 Jahreswert von 91,5 % und einem 13 Jahreswert von 89,1 %. Es wurden 25 Wechseloperationen, 1 Arthrodese, 1 Ankylose bei in situ verbliebener Prothese und 2 laterale Ergänzungen als Komplikationsfälle registriert, wobei 19 mal aseptische Lockerungen mindestens einer Prothesenkomponente im Vordergrund standen. Die Revisionsquote betrug 8,2 %Die subjektive Beurteilung des Operationsresultates durch den Patienten ergab in 90,7 % der Fälle sehr gute bzw. gute Ergebnisse, knapp 3 % der Patienten bezeichneten ihren postoperativen Zustand als mangelhaft. 298 (f: 226/m: 72) with a unicompartimental prosthesis supplied patients from the implantation period 1975 to 1995, could be examined in the context of this retrospective study in the orthopedic hospital in Ratingen. Post-operative 160,39 points. The knee score resulted in a postoperative value of 88,9 points, the function score of 71,49 points. The survival rates, computed according to the Kaplan-Meier method, express themselves in a 5-year survival of 96,1 %, a 10-year survival of 91,5 % and a 13-year survival of 89,1 %. 25 explantation revisions, 1 arthrodesis, 1 ankylosis with in situ remaining prosthesis and 2 lateral additions as cases of complication were registered, whereby 19 times non-septically loosening for at least one prosthesis component stood in the foreground. The revision ratio amounted to 8.2 %.The subjective evaluation of the result of operation by the patient showed in 90,7 % of the cases excellent or good results, scarcely 3 % of the patients called its post-operative condition unsatisfactory.
