dc.description.abstractDie akute Transplantatabstoßung ist eine wesentliche Ursache für Mortalität in den ersten Wochen nach Herztransplantation und beeinflusst Morbidität und Mortalität im Langzeitverlauf. Die akute Transplantatabstoßung ist charakterisiert durch die Infiltration von immunkompetenten mononukleären Zellen in das Transplantat. Es kommt zur Aktivierung von Adhäsionsmolekülen sowie zur Aktivierung verschiedener Zellpopulationen, die eine Reihe von Zytokinen exprimieren. In dieser Arbeit wurden Mediatoren der akuten Abstoßung in der Früh-, Intermediär- und der Spätphase der Abstoßung auf histologischer, immunhistochemischer und Genexpressionsebene im Modell der herztransplantierten Ratte analysiert (Zusammenfassung siehe Abbildung 21). Herzen von Wistar-Furth (WF) Spenderratten wurden heterotop in Lewis (LEW) Empfängerratten transplantiert. Die akute Transplantatabstoßung erfolgte innerhalb von 7 Tagen. Die Transplantate wurden 1, 3 und 6 Tage nach Herztransplantation untersucht. In der Standard H&E Färbung kam es histologisch zu einer progredienten Infiltration von mononukleären Zellen in das Transplantat. Bereits am Tag 1 nach Transplantation ist eine Hochregulierung der ICAM-1 und VCAM-1 Expression auf immunhistochemischer Ebene vor allem in postkapillären Venolen nachweisbar. Die Induktion dieser Adhäsionsmoleküle geht einher mit einer beginnenden zellulären Infiltration mit differenzierten alpha/beta-T-Zell Rezeptor positive Zellen (R73) sowie Monozyten und Makrophagen (ED1), die am Tag 6 ihr Maximum erreicht haben. In der semiquantitativen RT-PCR zeigt sich keine Expression von Adhäsionsmolekülen bei den nativen Kontrollherzen, dagegen kam es bei den transplantierten Herzen zu einem weiteren Anstieg der ICAM-1 und VCAM-1 mRNA Expression von Tag 1 bis zu Tag 3. In der Intermediärphase, an Tag 3 erreicht auch die Expression von IL-2 und MCP-1 mRNA ihr Maximum, während TNF-alpha und RANTES erst in der Spätphase der Abstoßung (Tag 6) maximal exprimiert werden. Zusätzlich kommt es in der Frühphase der akuten Abstoßung auf transkriptioneller Ebene zu einer maximalen Expression von IL-1 beta. Ebenso lässt sich der IL-2 Rezeptor, auf mRNA-Ebene schon bei den nativen Kontrollherzen nachweisen und steigt kontinuierlich bis zum Tag 6 nach Transplantation an. Die kinetische Charakterisierung von molekularen Mediatoren der akuten Transplantatabstoßung ermöglicht es, die akute Transplantatabstoßung hinsichtlich des Aktivierungsgrades von immunkompetenten Zellen in Endomyokardbiopsien über die rein histologisch und immunhistochemischen Methoden hinaus weiter zu kennzeichnen. Zusätzlich lassen sich in diesem definierten Tiermodell zeit- und zielgerichtete immunmodulierende Therapiestrategien evaluieren.de_DE
dc.description.abstractAcute cardiac allograft rejection is a substantial cause for mortality after heart transplantation and affects long term morbidity and mortality after successful transplantation. Clinical acute rejection is characterised by deterioration of graft function. Histologically it is characterised by progressive infiltration with immune competent mononuclear cells. This is mediated by the expression of adhesion molecules as well as secretion of cytokines. The aim of this study is to evaluate expression patterns of different adhesion molecules and cytokines in the early, intermediate and late face of acute cardiac allograft rejetion. Therefore, mediators of acute cardiac allograft rejection were analysed sequentially during the time course of rejection by histology, immunohistochemistry and gene expression profiling by RT-PCR in a heterotopic cardiac allograft model in the rat.Hearts of Wistar Furth (WF) donors rats were transplanted heterotropic to the infrarenal great vessels of Lewis (LEW) recipient rats. This is a fully allogeneic cardiac transplant model, leading to complete acute cardiac allograft rejection with cessation of graft function within 7 days. For further examination functioning allografts were removed on day 1, 3 and 6 after engraftment. In standard H&E histology a progressive infiltration with mononuclear cells could be detected during the time course within progression of acute allograft rejection. On day 1 after transplantation, upregulation of ICAM-1 and VCAM-1 expression was detected by immunohistology particularly on post-capillary venules. The expression of these adhesion molecules on endothelial cells, as a marker for the early phase of rejection, was accompanied by a progressive infiltration with alpha/beta T-cell-receptor-positive cells (R73) as well as ED-1 positive monocytes and macrophages. Maximal infiltration was observed on the day 6 after engraftment. Semiquantitative RT-PCR revealed no expression of adhesion molecules on the transcriptional level in naive control hearts, whereas ICAM-1 was detectable on day 1 after engraftment with a further rise at day 3 after engraftment. VCAM-1 was dectectable on day 3 after transplantation. As a marker for the intermediate phase of cardiac allograft rejection, IL-2 and MCP-1 mRNA expression was analysed. These cytokines reached their maximum expression on day 3 after transplantation. During the late phase of allograft rejection TNF-alpha and RANTES were evaluated on the transcriptional level. They reached their maximum expression on day 6 after engraftment. In addition during the early phase of acute cardiac allograft rejection on day 1 IL-1 beta reaches its maximum expression on the mRNA expression level. The IL-2 receptor is already expressed in naive control hearts and its expression rises continuously up to day 6 after transplantation. The kinetical characterization of molecular mediators during the time course of acute cardiac allograft rejection provides further opportunities to characterize acute rejection with regard to the degree of immunological activation of the infiltration immune competent cells to the bare histological or immunohistological characterization. Moreover it provides the framework for in vivo screening of immunosuppressive or immunomodulatory agents.en
