dc.description.abstractZiel der vorliegenden Studie war es, die Candida-Antigen- und Candida-Antikörperkonzentration sowie die Häufigkeit der Candidabesiedelung derMundhöhle und des Magen-Darmtraktes bei herzgesunden, immunsupprimiertenund/oder endokarditisprophylaxepflichtigen Kindern in Relation zum Kariesstatus zu eruieren. Die Genehmigung der Ethik-Kommission des FB-Medizin wurde am02.09.2004 erteilt. Es nahmen insgesamt 78 Kinder, 31 HTX- (GIII), 24 CHD- (GII) und 23herzgesunde Kinder (GI / Vergleichsgruppe), im Alter zwischen 2 und 16 Jahren mit Einverständnis der Eltern teil. Speichel-, Plaque-, Stuhl- und - wenn vorhandenauch kariös erweichte Zahnhartsubstanzproben wurden mittels Sabouraud- und CHROMTM-Agar und AUXACOLORTM2 Testsystem mikrobiologisch auf Keime derphysiologischen Mundflora, Streptococcus mutans, Laktobazillen und CandidaSpezies untersucht und weiterhin die Candida-Antigen/Antikörper - Quantität mittels PLATELIA® - Candida AG/AK-Test bestimmt. Der kariöse Gebissbefund wurdeanhand des DMF/dmf(T/t)-Index erfasst. Als Kariesgruppen wurden festgelegt:DMF/dmf(T/t)=0 (naturgesund); DMF/dmf(T/t) 7 1 (karieserfahren); D/d 7 1 (aktuelle Karies). Bei den diagnostizierten Candida Spezies gehörten von insgesamt 69 positivgetesteten Candida Proben 52 (75,4%) der Spezies C. albicans an. Weiter waren C.krusei, C. glabrata, C. lusitaniae, C. dubliniensis und C. parapsilosis vertreten. Zwischen den Patientengruppen bestand kein signifikanter Unterschied bei derHäufigkeit der Candidabesiedelung der Mundhöhle. Demgegenüber unterschiedsich die Candidabesiedelung der Stuhlproben mit p=0,027 signifikant, wobei Kinder in der GIII seltener Candida (9,7%) aufwiesen als in der GII (29,2%) und der GI (43,5%). Ferner belegen die Ergebnisse eine positive Korrelation (p<0,001) zwischen dem Auftreten einer Candidabesiedelung im Speichel und dem Kariesstatus der Zähne; selbiges wurde auch zwischen einer Candidabesiedelung der Plaque und demKariesstatus (p<0,001) gefunden. Die Häufigkeit für Candida nahm für die HTx-Kinder (GIII) in der Mundhöhle von 28,6% (naturgesund), über 66,7%(karieserfahren) auf 100% (aktuelle Karies) zu. Die Ergebnisse zeigen auch, dass eine positive Candida-Antigenkonzentration im Serum in 13% der Fälle für die Vergleichsgruppe, in 5,5% für die GII und in 16,1% für die HTx-Gruppe (GIII) bestand. Anders verhielt es sich bei der Candida- Antikörperkonzentration. Hier wiesen 13% der Kinder der Vergleichsgruppe und 16,7% der CHD-Gruppe eine positive Antikörperkonzentration auf, während in der HTx-Gruppe dies in keinem Fall nachgewiesen werden konnte. Letzteres ließ sich auf die noch unvermeidliche Immunsuppression bei den herztransplantierten Kindern zurückführen. Insgesamt lässt sich schließen, dass eine deszendierende Candidabesiedelung aus der Mundhöhle über den Gastrointestinaltrakt auch zu einer Candiahäufung imSerum führen kann. Insofern stellt die Vermeidung bzw. frühe Behandlung kariöserZähne eine wichtige Voraussetzung zur Prävention von Candidabesiedelungenjedweder Art im menschlichen Organismus dar. Dies gilt dann insbesondere fürherztransplantierte Kinder, da bei ihnen in Fällen von Candida-Antigen im Serum die Bildung der Antikörper unterbleibt und aufgrund der Immunsuppression die Gefahr besteht, dass die Diagnose einer invasiven Candidose erst zu spät gestellt oder nicht ausgeschlossen werden kann.de_DE
dc.description.abstractThe aim of this study was to determine the serum concentration of Candidaantigen/- antibody and the incidence of oral and intestinal Candida colonisation in children with healthy hearts, in immunosuppressed children and in children with congenital heart disease according to their dental status. The permission from the ethics committee of the Faculty of Medicine (University Giessen) is dated 2nd September 2004. A total of 78 children aged between 2 and 16 years were examined, including 31 HTx- (GIII), 24 CHD- (GII) and 23 children with healthy hearts (GI / comparison group). Written consent was obtained from the parents. Mutans streptococcus,lactobacilli, specimen from physiological oral flora and Candida species in saliva, dental plaque, carious lesions (if any) and stool were detected with conventional microbiological SABOURAUD- /CHROMagar and with the AUXACOLORTM2 system. The commercial PLATELIA® - Candida-specific antigen/antibody assays were used for serological diagnosis. We then determined the dental status on the basis of the DMF/dmf(T/t)-index on the following caries categories: DMF/dmf(T/t)=0 (healthy dentition); DMF/dmf(T/t) 71 (rehabilitated dentition); D/d(T/t) 71 (carious dentition). Candida albicans was the most detected yeast with 52 (75,4%) of 69 Candidaalbicans samples testing positive. Microbiological analysis revealed several Candida species, such as Candida albicans, Candida dubliniensis, Candida glabrata, Candida krusei and Candia lusitaniae. There was no significant difference between the three groups in terms of Candida colonisation in the oral cavity. On the other hand, there were significant differences (p=0,027) between the groups in terms of Candida colonisation in stool specimens; here, group III had the fewest Candida colonisation in stool specimens (9,7%) compared to GII (29,2%) and GIII (43,5%).Furthermore, significant correlations with p<0,001 between the incidence of Candida in saliva and dental status and between plaque and dental status were indicated. In relation to dental status, oral Candida colonisation increased within group III from 28,5% (healthy dentition) to 66,7% (rehabilitated dentition) up to 100%(carious dentition). Our results show, that Candida-antigen concentration was determinate positive in 16,1% (HTx), 5,5% (CHD) and 13% (comparison group) other than the Candidaantibody concentration. Here, a positive Candida-antibody concentration was diagnosed in 13% of cases in the comparison group and 16,7% of patients in the CHD group but not in group III (HTx), obviously as a consequence of theimmunosuppression. We assume that high oral Candida and their descending/resorption over thegastrointestinal tract may lead to serological Candida accumulation. Therefore,healthy oral cavity is an important precondition for preventing any Candidacolonisation in vivo. This applies especially in children after HTx, because in cases of positive Candida-antigen concentration there is no Candida-antibodysaccumulation due to their immunosuppression. Therefore, an invasive candidiasis is diagnosed too late or cannot be excluded.en
