| dc.description.abstract | Die Anwendung endoskopisch bildgebender Verfahren zur Diagnosefindung in der Gastroenterologie ist heute unerlässlich.Bei der Verwaltung, Befundung, Dokumentation und Archivierung des vermehrt anfallenden Bild- und Datenmaterials hat sich der Computer konventionellen Möglichkeiten als überlegen erwiesen.Im Rahmen der Studie soll untersucht werden, ob eine Dokumentation einer kompletten Oesophagogastroduodenoskopie mittels filmischer Aufzeichnung einer reinen Standbilddokumentation überlegen ist, und ob dies ökonomisch vertretbar ist.Ein weiterer Aspekt, der untersucht werden soll ist, ob die konsequente Nutzung eines Dokumentationssystems mit strukturierter Nomenklatur die Vollständigkeit des Befundes beeinflusst und die Qualität der Befundung beim Vergleich eines unerfahrenen Untersuchers mit einem erfahrenen Endoskopiker.Hierzu erfolgte nach Durchführung von 101 Oesophagogastroduodenoskopien an 97 Patienten die statistische Auswertung der Speicherkapazitäten und der Befundübereinstimmungen zwischen Originalbefunden und Film- bzw. Bildnachbefund.Aus den Ergebnissen lässt sich schliessen, dass die Filmdokumentation bei Rückzug des Endoskopes gut zur Dokumentation eines endoskopischen Befundes geeignet ist.Anhand des Films ist eine Zweitbefundung möglich, die nicht nur bereits in der Originaluntersuchung aufgefallene Auffälligkeiten nachvollsziehbar macht, sonsderndarüber hinaus die Diagnose zusätzlicher Auffälligkeiten ermöglicht.Die Filmdokumentation übertrifft die Standbilddokumentation hinsichtlich Vollständigkeit und ist weniger abhängig vom Originaluntersucher.Trotz der verbesserten Dokumentationsmöglichkeiten und der andauernden Bemühungen um Standardisierung bleibt die Endoskopie weiter abhängig vom Untersucher, seinen Kenntnissen, seinen Fähigkeiten und Erwartungen. In diesem Rahmen kann die Filmdokumentation der Endoskopien, verbunden mit einer Nachbefundung durch der Erstuntersucher oder einen anderen Gastroenterologen, nicht nur der juristisch korrekten Dokumentation der Untersuchung dienen, sondern auch ein wichtiges Hilfsmittel zur weiteren Objektivierung der Befunde sein. Sie wird in Zukunft auch den Einsatz von diagnostischen Computerprogrammen zur Unterstützung der Befundung ermöglichen. | de_DE |

By management, medical findings, documentation and archiving the increasingly resulting picture and data material the computer to conventional possibilities has itself as considere prove.Within the scope of the study should be examined wether a documantation of a complete diagnostic gastroscopy by means of cinematic recording of a pure statue documentation consider is, and wether this is economically defensible.Another aspect hin should be examined, is influenced wether the logical use of a documentation system with structured nomenclature the completeness of the medical findings an the quality of the medical findings with the comparisonof an inexperienced investigator with an experienced endoscopist.Morever the statistical evaluation of the original film or the original statues picture occured after realisation of 101 diagnostic gastrocopies to 97 patients in comparison of the postfindings.Were judged here, in addition, storage capacity and findings between original findings and film- and picturepostfindings.The result of the study support the conclusion that the film images create during the withdrawal of an endoscope are very suitable for the documentation of an endoscopic diagnosis.the film material provides both the possibility of confirming the conclusions of the original examination and enables the detection of additionel abnormalities.The film documentation surpasses freeze-frame documentation in beeing more complete an less dependant on the subjective interpretation of the original examiner.Despite the continuing efforts to standardise the endoscopis procedures, the result still depend significantly on the examiner and his skills, knowledge and expectations. In this context the film documentation of the endoscopies in combination with the follow-up diagnosis by the original examiner or an another gastroenterologist could only serve to provide a legally correct documentation of the examination, but also be an important aid to increase the objectivity of diagnosis and will enable the use of an diagnostic computer programs in supporting diagnosis for the future. | en |
