dc.description.abstractIn der vorliegenden Studie wurde die Dimensionsgenauigkeit von Abformmaterialien bei der offenen und geschlossenen Implantatabformung in Zusammenhang mit den beiden Implantatsystemen Straumann (Freiburg) und BEGO (Bremen) untersucht. Die Besonderheit dieser Studie bestand zum einen in der Gegenüberstellung der beiden Implantatsysteme, da bislang noch kein direkter Vergleich unterschiedlicher Implantatsysteme in der Literatur beschrieben wurde. Zum anderen aber auch wurden in dieser Studie zusätzliche Parameter wie Neigungsänderung der Implantatachse, Rotation des Implantates und Verschiebung der Implantatmittelpunkte entlang der x- , y-, z-Achse bei der Übertragung der Implantatposition vom Urmodell auf das Gipsmodell untersucht.Dazu wurde ein auf einer Edelstahlplatte montiertes Oberkiefermodell aus Kunststoff angefertigt, welches lediglich Frontzähne von 13 bis 23 aufwies. Im Bereich des unbezahnten Kiefers wurden sechs in Edelstahlröhrchen fixierte Implantate eingebracht. Die Straumann-Implantate befanden sich in regio 14, 18 und 26, die BEGO-Implantate in regio 16, 24 und 28, so dass beide Implantatsysteme spiegelverkehrt zueinander angeordnet waren. Bei beiden Implantatsystemen wurde als offene Abformung die ungeschiente PickUp-Technik unter Verwendung eines individuellen Abformlöffels untersucht. Die geschlossene Abformung wurde mit einem konfektionierten Abformlöffel durchgeführt. Bei dem Implantatsystem BEGO wurde die Repositionstechnik verwendet. Hierzu wurden die Abformpfosten in die Transfer-Caps, die sich in der Abformung befanden, reponiert. Bei dem Implantatsystem Straumann kamen Plastikabformpfosten mit einem Snap-on-Mechanismus zur Anwendung, so dass diese Abformpfosten nach Entfernung des Löffels in der Abformung verblieben. Insgesamt wurden bezüglich der offenen und geschlossenen Abformung jeweils zwei Polyether (Impregum Penta von 3M ESPE, Seefeld und P2 Polyether Magnum von Heraeus-Kulzer, Hanau) und zwei A-Silikone (Flexitime Monophase von Heraeus-Kulzer, Hanau und Aquasil Ultra von Dentsply, Konstanz) als Abformmaterial getestet. Das Hybridmaterial Fusion von GC Dental (Leuven, Belgien) kam nur bei der offenen Abformung zur Anwendung, da es danach nicht mehr auf dem europäischen Markt erhältlich war. Die hergestellten Gipsmodelle wurden mit Hilfe der 3D-Koordinatenmessmaschine RAPID (THOME, Messel) und speziell angefertigten Vermessungspfosten vermessen. Die Ergebnisse der Abformmaterialien zeigten nicht signifikante Unterschiede, so dass sich die Verwendung von Polyethern oder A-Silikonen als Abformmaterial nur geringfügig auf die Dimensionsgenauigkeit bei einer Einphasenimplantatabformung auswirkte. Dagegen fielen bei dem Vergleich der offenen mit der geschlossen Abformung bei beiden Implantatsystemen signifikante Unterschiede in Zusammenhang mit der Neigung und der räumlichen Verschiebung auf. Insgesamt war die offene Abformung bei dem Implantatsystem BEGO dimensionsgenauer als die geschlossene Abformung mit der Repositionstechnik. Anders hierzu verhielten sich die geschlossene und die offene Abformtechnik des Implantatsystems Straumann. Hierbei zeigte insgesamt die geschlossene Abformung mit dem Snap-on-Mechanismus bessere Ergebnisse in der Dimensionsgenauigkeit einer Implantatabformung. Auch die Verwendung des Implantatsystems wirkte sich in dieser Studie auf die Abformgenauigkeit aus, da mit Hilfe des Implantatsystems BEGO eine höhere Dimensionsgenauigkeit vor allem in Zusammenhang mit der offenen Abformung erzielt werden konnte. Insgesamt hängt die Dimensionsgenauigkeit einer Implantatabformung stark von der angewendeten Abformtechnik ab. Auch das verwendete Implantatsystem trägt zu einer höheren Abformgenauigkeit bei. Dagegen hat die Verwendung von A-Silikonen, Polyethern oder Hybridmaterialien als Abformmaterial keinen Einfluss auf die Dimensionsgenauigkeit einer Implantatabformung.de_DE
dc.description.abstractIn this study the dimensional accuracy of impression material of open and closed implant impressions with the two implant systems Straumann (Freiburg) and BEGO (Bremen) was examined. The distinctive feature of this study, on one hand, consisted of the comparison of both implant systems, because, until now, no direct comparison of different implant systems was described in literature. On the other hand, this study examines additional parameters such as the tilt of the implant axis, the rotation of the implant and the shifting of the implant centres along the x-, y-, z-axis during the transmission of the implant position from the master form to a Type IV stone cast. For that reason an upper jaw model made of acrylic resin and installed on a plate made of stainless steel was fabricated, which only contained the front teeth from 13 to 23. In the area of the toothless jaw, six implants fixed in small pipes made of stainless steel were inserted. The Straumann implants located in regions 14, 18 and 26 and the BEGO implants in regions 16, 24 and 28, so that both implant systems were arranged mirror inverted to each other. On both systems the non splinted PickUp technique, as open impression, was examined using a custom tray. The closed impression was carried out with a stock tray. The reposition technique was used for the implant system BEGO. For this the transfer copings were seated into the transfer caps, that were trapped in the impression. Plastic transfer copings with a snap-on-mechanism were used for the Straumann implant system, with the result that the transfer copings remained in the impression after the removal of the tray. On the whole, referring to the open and closed impression, two polyethers (Impregum Penta by 3M ESPE, Seefeld and P2 Polyether Magnum by Heraeus-Kulzer, Hanau) and two polyvinyl siloxane (Flexitime Monophase by Heraeus-Kulzer, Hanau and Aquasil Ultra by Dentsply, Konstanz) were tested as impression material. The hybrid material Fusion by GC Dental (Leuven, Belgium) was only used for the open impression, as it was later no longer available on the European market. The accuracy of the working cast was measured with a 3D-coordinate measuring machine RAPID (THOME, Messel) and specially made measuring ports.The results for the impression materials did not show any significant differences so that the use of either polyethers or polyvinyl siloxane as impression material only marginally affected the dimensional accuracy during a one-phase implant impression. Significant differences could be observed for the open and closed impression technique using both implant systems. On the whole the open impression technique with the implant system BEGO was dimensionally more precise than the closed impression with the reposition technique. The open and closed impression of the Straumann implant system, however, showed different results. Here, the closed impression with the snap-on-mechanism showed better results concerning the dimensional accuracy of an implant impression. Also the use of implant systems affected the accuracy of impression within this study, as by using the BEGO implant system, a higher dimensional accuracy was achieved especially with regard to the open impression.On the whole the dimensional accuracy of an implant impression depends on the impression technique applied as well as selected. In contrast the use of polyvinyl siloxane, polyethers or hybrid materials as impression material has no influence on the dimensional accuracy of an implant impression.en
