dc.description.abstractDas zu der Gruppe der Kopf-Hals-Tumoren gehörende Nasopharynxkarzinom (NPC) hat eine hohe Inzidenz in Südostchina und ist mit etwa 70% der häufigste maligne Tumor im Bereich des Pharynx. Insgesamt ist das NPC jedoch in Mitteleuropa ein seltener Tumor mit einem Gesamtanteil von 0,2% aller Tumorerkrankungen und macht in der Kindheit nur etwa 1% aller Malignome aus. Neben verschiedenen Umweltfaktoren, Ernährungsgewohnheiten und genetische Faktoren wie z. B. die Assoziation mit HLA-A2, HLA-B17 und HLA-Bw46 wird insbesondere die Infektion der Zellen mit dem Epstein-Barr-Virus (EBV) als Ursache für die Entstehung des NPC gesehen. Bei nahezu allen Patienten mit einem NPC konnte EBV-DNA im Plasma nachgewiesen werden. Ebenso zeigte sich, dass es eine Korrelation zwischen der Konzentration von EBV-DNA im Plasma und dem Tumorstadium, der Rezidivwahrscheinlichkeit, des Überlebens sowie das Vorhandensein von Resterkrankung nach einer Strahlentherapie bei Patienten mit NPC gibt. In dieser Arbeit konnte ebenfalls ein Zusammenhang zwischen dem Auftreten von Nasopharynxkarzinomen und dem Nachweis von Plasma-EBV-DNA festgestellt werden. Erhöhte Konzentrationen an Plasma-EBV-DNA lassen sich bei Patienten mit einem NPC nachweisen, welche bereits während der Behandlung gemäß dem NPC-2003-GPOH-Studienprotokoll abnehmen und nach Beendigung der Therapie kaum noch nachweisbar sind. Demzufolge eignet sich die Bestimmung von Plasma-EBVDNA zur Überwachung von NPC-Patienten.Die Bestimmung von EBV-DNA in den PBMCs ist zwar zur Überwachung der pädiatrischen Patienten nach einer allogenen Stammzell- oder Organtransplantation brauchbar, nicht aber zur Überwachung von NPC-Patienten. Des Weiteren wurde in dieser Arbeit gezeigt, dass die EBV-Serologie zur Überwachung des Therapieansprechens sowie der Früherkennung eines Rezidives bei pädiatrischen Patienten mit NPC weniger geeignet ist.de_DE
dc.description.abstractNasopharyngeal carcinoma (NPC), which has a high incidence in southeast China and belongs to the group of head and neck cancer, is about 70% of the most common malignant tumor in the region of the pharynx. Overall, but NPC is a rare tumor in Central Europe with a total amount of 0.2% of all tumors and makes only about 1% of all malignancies in childhood. In addition to various environmental factors, diet and genetic factors such as the association with HLA-A2, HLA-B17 and HLA-Bw46 in particular the infection of the cells with Epstein-Barr virus (EBV) is a causative factor for the development of NPC. In almost all patients with NPC EBV DNA is detectable in plasma. Similarly, it was found that there is a correlation between the concentration of EBV DNA in plasma and tumor stage, recurrence probability of survival as well as the presence of residual disease after radiotherapy in patients with NPC. In this work, we demonstrated a correlation between the activity of nasopharyngeal carcinoma and plasma EBV DNA. Elevated concentration of plasma EBV-DNA was detected in pediatric patients with NPC, which decreased during the treatment according to the NPC-2003 GPOH study protocol and barely detectable after cessation of therapy. Therefore, the determination of plasma EBV-DNA is suitable for monitoring of NPC patients.The determination of EBV-DNA in the PBMCs is although useful for monitoring of pediatric patients after an allogenic stem cell transplantation and solid organ transplantation, but not for monitoring of the NPC patients. Furthermore, EBV serology is less suitable for the monitoring of treatment response as well as early detection of recurrence of pediatric patients with NPC.en
