dc.description.abstractEndometriose bezeichnet das pathologische Vorkommen von Endometrium außerhalb des Cavum uteri und ist eine der häufigsten gynäkologischen Erkrankungen im repro- duktiven Alter. Je nach Lokalisation des ektopen Endometriums kann man diese Er-krankung in unterschiedliche Formen einteilen. Bei einer Infiltration von mehr als 5 mm unterhalb des Peritoneums liegt eine tief infiltrierende Endometriose vor, im Gegensatz dazu gibt es eine peritoneale und ovarielle Endometriose sowie die Adenomyose, bei der das Myometrium des Uterus betroffen ist. Bei der Diagnosefindung helfen bildgebende Verfahren wie transrektaler, transvaginaler oder abdomineller Ultraschall sowie das MRT, alternativ ist eine diagnostische Laparoskopie möglich.Ziel dieser Studie war die Evaluation der Genauigkeit des MRT in der Endometriosediagnostik, indem MRT-Befunde mit Operationsbericht und Pathologiebefund hinsichtlich des Vorkommens und der Ausdehnung der Endometriose korreliert wurden.Bei falsch negativem Befund im MRT erfolgte unter Kenntnis des Operations- und Pathologieberichtes eine erneute Auswertung der MRT-Bilder.Es wurden 131 Patientinnen in die Studie eingeschlossen, bei denen klinisch der Verdacht auf Endometriose bestand und präoperativ eine MRT-Untersuchung des Beckens mit einem 1,5-Tesla-Gerät durchgeführt wurde. Das MRT-Protokoll beinhaltete eine native T1-gewichtete transversale Sequenz mit Fettunterdrückung sowie T2-gewichtete sagittale, transversale und koronare Sequenzen sowie T1-gewichtete Aufnahmen nach Kontrastmittelgabe. Das Becken wurde in drei Kompartimente unterteilt: das vordere Kompartiment mit Blase, Ureter und Uterusvorderwand; das mittlere Kompartiment mit Vagina, Uterus, Ovarien, Ligamenta teres uteri sowie Tuben und das hintere Kompartiment mit Douglasraum, uterosakralen Ligamenten (USL) und Intestinaltrakt. Endometrioseherde im Bereich von Beckenwand, Peritoneum und Bauchdecke sowie Adhäsionen wurden unter sonstigen Befunden zusammengefasst. Zusätzlich erfolgte eine getrennte Auswertung von Befunden der tief infiltrierenden Endometriose mit = 5 mm sowie oberflächlichen Läsionen < 5 mm.Die mittlere Größe der Läsionen betrug im vorderen Kompartiment 1,8 cm, im mittleren Kompartiment 3,4 cm, im hinteren Kompartiment 2,2 cm und bei den sonstigen Befunden 1,3 cm. Die Sensitivität für alle Befunde bzw. für die tief infiltrierende Endometriose und die Spezifität betrugen für Harnblase 54,2 % (13 von 24) bzw. 54,6 % (12 von 10) und 99,1 % (106 von 107); Uterus 61,3 % (19 von 31) und 96 % (96 von 100); Ovarien 83,3% (45 von 54) bzw. 87,5 % (42 von 48) und 80,5 % (62 von 77); Douglas-Raum 83,3 % (15 von 18) bzw. 86,7 % (13 von 15) und 98,2 % (111 von 113); USL 64,5 % (20 von 31) bzw. 65,4 % (17 von 26) und 98 % (98 von 100); Rektosigmoid 90,2 % (37 von 41) bzw. 88,6 % (31 von 35) und 97,8 % (88 von 90); Obliteration des Douglas-Raums 96,3 % (52 von 54) und 98,7 % (76 von 77); Adhäsionen 98,6 % (71 von 72) und 93,2 % (55 von 59). Insgesamt wurde eine Sensitivität von 91,7 % (110 von 120) für alle Befunde bzw. 94,9 % (92 von 97) für die tief infiltrierende Endometriose und eine Spezifität von 90,9 % (10 von 11) erzielt.Die Ergebnisse legten den Schluss nahe, dass die Aussagekraft des MRT bei der Diagnose der Endometriose abhängig von der Größe ist. Im vorderen Kompartiment konnten vermutlich aufgrund der geringen Größe nur etwas mehr als die Hälfte der Endometrioseläsionen im MRT erkannt werden. Die Stärke des MRT lag in der vorliegenden Studie in der Diagnose von Läsionen im mittleren und hinteren Kompartiment, bei denen eine hohe Treffsicherheit erzielt werden konnte. Des Weiteren konnte im mittleren Kompartiment, in dem die Endometrioseläsionen durchschnittlich am größten waren, die höchste Genauigkeit des MRT in deren Detektion erreicht werden. Befunde der oberflächlichen Endometriose < 5 mm konnten nur in einem Drittel der Fälle im MRT erkannt werden, jedoch schränkt die geringe Fallzahl in der vorliegenden Studie die Aussagekraft der Ergebnisse ein.Eine weitere Stärke des MRT lag in der Diagnose von Adhäsionen, während die übrigen sonstigen Befunde nur in einem Drittel der Fälle detektiert werden konnten, wobei möglicherweise ihre geringe Größe eine Rolle spielte.Insgesamt wurden 8 von 10 Patientinnen in der Bildgebung korrekt diagnostiziert, sodass die Magnetresonanztomographie sehr gut zur präoperativen Diagnostik bei Verdacht auf Endometriose eingesetzt werden kann, sofern man sich ihrer Limitationen bewusst ist.de_DE
dc.description.abstractEndometriosis, defined as the presence of ectopic endometrial implants outside the uterine cavity, is a common gynaecological disorder in women of reproductive age. There are different types of endometriosis depending on localisation of the implants. Deep infiltrating endometriosis is diagnosed if lesions penetrate the peritoneum of more than 5 mm in depth. Additionally women can be affected by superficial peritoneal lesions, ovarian endometriosis and adenomyosis, in which endometrial glands and stroma infiltrate the myometrium. Imaging modalities for diagnosing endometriosis include transvaginal ultrasound, transrectal endoscopic ultrasound and MRI, alternatively diagnostic laparoscopy.The aim of this study was to investigate the value of MRI in the preoperative diagnosis of endometriosis by correlating results of MRI with laparoscopic and histopathologic findings. In the case of false negative results MRI images were re-evaluated.131 patients with clinical suspicion of endometriosis underwent MRI followed by surgial and histopathological investigations. MRI at 1.5 Tesla was performed with T1-weighted, fat-suppressed axial images, T2-weighted sagittal, axial and coronar images and T1-weighted images using intravenous contrast medium. The pelvis was divided into three compartments: the anterior compartment containing bladder, ureter and front side of the uterus; the middle compartment containing vagina, uterus, ovaries, ligamenta teres uteri and ovarian tubes; the posterior compartment containing the pouch of Douglas, uterosacral ligaments (USL) and rectosigmoid colon. Further endometrial implants affecting the wall of the pelvis, peritoneum, abdominal wall and adhesions were sum-marized as other manifestations. In addition deep infiltrating endometriosis and superficial lesions were separately evaluated. The mean size of lesions measured 1.82 cm in the anterior compartment, 3.35 cm in the middle compartment, 2.23 cm in the posterior compartment and 1.25 cm for other manifestations. The sensitivity and specifity of MRI for endometriosis in all sites were 91.7 (110 of 120), 94,9 % (92 of 97) for deep infiltrating endometriosis and 90.9 (10 out of 11), respectively. The sensitivity for the diagnosis of endometriosis, deep infiltrating endometriosis and specifity in specific sites were as follows: bladder 54.2 % (13 out of 24), 54,6 % (12 of 10) and 99.1 % (106 out of 107); uterus 61.3 % (19 out of 31) and 96% (96 out of 100); ovaries 83,3 % (45 out of 54), 87,5 % (42 of 48) and 80.5 % (62 out of 77); pouch of douglas 83.3 (15 out of 18), 86,7 % (13 von 15) and 98.2 % (111 out of 113); USL 64.5 % (20 out of 31), 65,4 % (17 von 26) and 98 % (98 out of 100); rectosigmoid colon 90.2 % (37 out of 41), 88,6 % (31 von 35) and 97.8 % (88 out of 90); obliteration of the pouch of Douglas 96.3 % (52 out of 54) and 98.7 % (76 out of 77); adhesions 98.6 % (71 out of 72) and 93.2 % (55 out of 59).In conclusion, the value of MRI in diagnosis of endometriosis depends on the size of endometrial implants. Lesions in the anterior compartment were seen in MRI in only about half of all cases. Compared to other compartments the mean size of lesions was smaller. Both the middle and posterior compartment showed high accuracy for diagnosis of endometriosis. The largest mean diameter of implants was found in the middle compartment in which the highest accuracy was detected. Superficial lesions could only be seen in one third of the cases. However results may have limited significance due to the small number of lesions < 5 mm.Furthermore, the diagnosis of adhesions proved to be one of MRI´s strength, other manifestations could only be seen in one third of all cases which may be the case due to their small diameters.All in all 8 out of 10 patients could be correctly diagnosed using MRI, thus, acknowledging its limitations, this imaging modality seems to be a very useful preoperative test for patients with suspicion of endometriosis.en
