dc.description.abstractDie Implantat-Prothetik gewinnt seit Jahren immer mehr an Bedeutung in der Zahnmedizin. Es gibt jedoch bisher nur wenige Studien, die sich sowohl mit der klinischen Bewährung von festsitzenden, als auch mit der klinischen Bewährung von herausnehmbaren implantatgetragenen Suprakonstruktionen unter patientenspezifischen Faktoren, sowie auftretenden prothetischen Komplikationen und dadurch anfallenden Nachsorgemaßnahmen über einen längeren Zeitraum befasst haben.Die vorliegende retrospektive Longitudinalstudie basiert auf Daten von 473 Patienten, welche im Zeitraum von Dezember 2003 bis Februar 2017 implantatgetragenen Zahnersatz in der Abteilung für Zahnärztliche Prothetik des Zentrums für Zahn-, Mund- und Kieferheilkunde der JLU Gießen erhielten. Hierbei wurden außerdem Patienten mit einer die Mundhöhle betreffenden Tumorerkrankung in die Studie miteinbezogen. Dabei wurde randomisiert nur eine implantatgetragene Versorgung pro Patient ermittelt, sodass schließlich 320 festsitzende und 153 herausnehmbare implantatgetragene Versorgungen auf insgesamt 1499 Implantaten untersucht wurden. Die statistische Auswertung erfolgte unter anderem mittels der Kaplan-Meier-Analyse und Cox-Regression.Während der beobachteten Funktionsperioden mussten 31 (6,6%) von insgesamt 473 Versorgungen neuangefertigt werden. Betroffen waren 24 (7,5%) festsitzende Versorgungen und sieben (4,6%) herausnehmbare Versorgungen. Die kumulative 5- Jahres-Überlebensrate lag bei festsitzenden Konstruktionen bei 87,4% und bei herausnehmbaren bei 95,5%. Die kumulative Überlebenswahrscheinlichkeit nach zehn Jahren in Funktion lag bei 69,6% bzw. bei 90,2%. Die 90%-ige- Überlebenswahrscheinlichkeit wurde bei festsitzenden Suprakonstruktionen nach 4,7 Jahren unterschritten, bei herausnehmbaren nach 11,4 Jahren.Als Gründe für notwendige Neuanfertigungen wurden vor allem Implantatverluste (8) und Verblendungsbrüche (7) dokumentiert. Weitere Gründe waren unter anderem ästhetische Mängel (3), Frakturen der Abutmentschrauben (2), der Verlust von Suprakonstruktionen (3), Friktionsverlust (1), u.a. (7).Hinsichtlich der einbezogenen patientenspezifischen Faktoren zeigte sich bei den festsitzenden Suprakonstruktionen ein signifikanter Einfluss in Abhängigkeit von der Gegenbezahnung (p=0,05), wobei sich eine kürzere Überlebensdauer bei Versorgungen mit ebenfalls implantatgetragenen Versorgungen im Gegenkiefer zeigte. Beim Patientenkollektiv mit herausnehmbaren implantatgetragenen Suprakonstruktionen erwies sich die regelmäßige Recallteilnahme als signifikante Variable (p<0,05). Patienten, die regelmäßig das Recallprogramm, sprich die Kontrolluntersuchungen in Anspruch nahmen, konnten eine signifikant längere Überlebenszeit des vorhandenen implantatgetragenen Zahnersatzes von 12 Jahren im Gegensatz zu 7,9 Jahren bei Nicht- teilnahme am Recallprogramm erzielen. Eine vorhandene Tumorerkrankung des Patienten erwies sich bei beiden Versorgungsarten als nicht signifikant hinsichtlich der Überlebensrate des implantatgestützten Zahnersatzes.Während der betrachteten Funktionsperiode mussten bei 53,1% der Patienten 661 prothetische Nachsorgemaßnahmen durchgeführt werden. Die Verweildauer bis zur ersten notwendigen prothetischen Nachsorgemaßnahme war bei den Patienten mit herausnehmbaren Versorgungen höchst signifikant kürzer als bei solchen mit festsitzenden Versorgungen (p<0,001). Im Vergleich lag die 5-Jahres-Verweildauer des herausnehmbaren implantatgetragenen Zahnersatzes (HIZE) bei 17,6% im Gegensatz zu 39,4% bei festsitzendem implantatgetragenem Zahnersatz (FIZE).Die häufigste Nachsorgemaßnahme bezogen auf HIZE war hierbei die Druckstellenentfernung (32,7%), gefolgt von Friktionsverbesserungen und Unterfütterungen (jeweils 13,6%). Bei FIZE war die mit Abstand häufigste prothetische Nachsorgemaßnahme das Rezementieren von Suprakonstruktionen (44,9%), gefolgt von Lockerungen der Verbindungsschrauben (18,3%) und Verschlusserneuerungen (14,8%).de_DE
dc.description.abstractThe field of implant prosthetics has continuously been gaining importance in dentistry in recent years. However, there are only few studies which investigate the successful use of fixed and removable implant-retained dental prostheses under the influence of various patient-related modelling factors as well as occuring prosthetic complications with associated necessary aftercare-measures over an extended time period.Included in this retrospective longitudinal study, are the data of 473 patients who received dental prostheses at the Department of Prosthodontics at the Dental Clinic in Giessen between December 2003 and February 2017. Of these 473 patients, 320 received fixed and 153 received removable implant implant-supported dental prostheses based on a total of 1499 implants. This study additionally includes patients with diagnosed forms of oral cancer. Only one implant-supported treatment was randomly chosen for each patient. Statistical analysis was performed by using Kaplan-Meier-Analysis and Cox-Regression amongst others.During the observational period, 31 (6.6%) of the 473 supraconstructions had to be renewed. Of these, 24 (7.5%) were fixed while 7 (4,6%) were removable supraconstructions. The cumulative 5-year probability of survival was 87.4% for fixed implant-retained tooth restorations and 95.5% for removable implant-retained restorations.After ten years in use, the cumulative probability of survival was 69.9% and 90.2% respectively. For fixed supraconstructions it took 4.7 years for the probability of survival to fall below 90% whereas this took 11.4 years for removable supraconstructions. Documented reasons for neccessary renewals were loss of implants (8) and irreparable fractures of the veneers (7)in particular. Further reasons were aesthetic deficits (3), fractures of abutment screws (2), loss of supraconstructions (3), loss of friction (1), and other (7).Within the group of patients with fixed supraconstructions, the opposing dentition showed a significant influence on the survival time of the fixed restorations (p=0/05). Implant- supported fixed restorations with implant-supported restorations in the opposing jaw exhibited a shorter survival time.Concerning the group with removable implant-retained prostheses, patient participation in the recall programme had a significant impact on the supraconstruction´s survival time (p<0.05). The survival time of the implant-retained supraconstructions of patients who participated in the programme was 12 years, as opposed to a survival time of 7.9 years for non-participating patients.Present tumour diseases of the oral cavity did not significantly impact on the survival time of supraconstructions.During the observation period, 53.1% of patients required a total of 661 aftercare- measures for their implant-retained dental prostheses. The cumulative survival time until the first required prosthetic aftercare-measure was significantly lower for patients with removable supraconstructions compared to those with fixed ones (p<0.001). The cumulative 5-year-survival time until the first required prosthetic aftercare-measure was 17.6% for removable and 39.4% for fixed implant-supported dental prostheses.The most common aftercare-measure concerning removable supraconstructions was the removal of sore spots (32/7%), followed by friction adjustments and relining of the prostheses (each 13/6%). Looking at fixed supraconstructions, the most common prosthetic aftercare-measure was recementing of supraconstructions (44.9%), followed by loosening of the connecting screw (18.3%) and renewal of occlusal composite cap (14.8%).en
