Purpose of this work was to clarify the feasibility of using Deep Learning algorithms in classifying lung CT-scans into healthy and fibrosis. More detailed, the influence of different AI algorithms with respect to specificity and sensitivity was investigated in a simple classification task between healthy and fibrotic CT lung scans. Additionally, the influence of different selection rules of training, validation and test groups was evaluated. In this study, the dataset consisted of high-resolution CT-scans of 52 patients for each classification group. The models were trained, validated and tested with these data. Five different models were evaluated.First model: First, randomly selected images were selected from the folders "Gesund" and "Fibrose" and copied to the corresponding folders "Lehrdaten_Gesund" (1351 images), "Lehrdaten_Fibrose" (942 images), "Prüfdaten_Gesund" (1169 images) and "Prüfdaten_Fibrose" (1057 images). With this image specific distribution of the data, the neural network Inception v3 was retrained with TensorFlow.Second model: In a second training process, more image data was included in the learning and examination process. In addition, a patient-specific division of the data was performed. Twenty-six randomly selected patients were copied into the folders "Lehrdaten_Gesund" (5622 images), "Lehrdaten_Fibrose" (3323 images), "Prüfdaten_Gesund" (5407 images) and "Prüfdaten_Fibrose" (2919 images). With this patient-specific division, Inception v3 was retrained with TensorFlow.Third to 5th model: Additionally, the neural networks DenseNet121, Inception v3 and ResNet50 were retrained using the program library PyTorch. For this purpose, patient- and diagnosis-specific data were divided into the folders "Lerndaten_Gesund" (23 patients), "Validierungsdaten_Gesund" (3 patients), "Prüfdaten_Gesund" (26 patients), "Lerndaten_Fibrose" (23 patients), "Validierungsdaten_Fibrose" (3 patients) and "Prüfdaten_Fibrose" (26 patients). Class-Activation-Maps were generated for some selected images to visualize the basis of the classification decision of the software. The retrained neural networks were successful in classifying lung CT-scans into healthy and fibrosis. In the first image-specific distribution of the data, the neuronal network Inception v3 (TensorFlow) performed with a sensitivity of 96.9 % and a specificity of 95.4 %. The patient-specific distribution of the data led to a sensitivity of 99.1 % and a specificity of 99.4% (TensorFlow). DenseNet121 (PyTorch) and Inception v3 (PyTorch) correctly classified all test data, showing a sensitivity and a specificity of 100 %. ResNet50 (PyTorch) classified two fibrotic images as healthy (sensitivity 99.93 %, specificity 100 %).These findings suggest that Deep learning methods are robust and suitable for a fully automated classification of CT thoracic images into "healthy" and "fibrosis". Further studies are required to evaluate an automated image classification performance into different fibrosis severity grades and their corresponding morphological patterns.en
