Untersuchungen zur Cefquinomkonzentration im Blut und im Bauchraum bei laparotomierten Pferden

Abstract

Ziel der vorliegenden Arbeit war es, die Cefquinomkonzentrationen im Blut und in der Bauchhöhlenflüssigkeit im Zusammenhang mit einer präventiven Behandlung der postoperativen Bauchhöhlenentzündung bei laparotomierten Pferden zu bestimmen. Schließlich folgte der Vergleich zwischen den Ergebnissen einer alleinigen systemischen Antibiotikatherapie und den Ergebnissen einer systemischen Antibiotikatherapie mit zusätzlicher lokaler intraperitonealer Antibiotikatherapie. Es wurden verschiedene klinische als auch Laborparameter verglichen.Die Ergebnisse der vorliegenden Studie belegen, dass nach einmaligen systemischen i.v. Applikation von Cefquinom in der vom Hersteller empfohlenen Dosierung einer wirksamen Antibiotika-Konzentration sowohl im Blut als auch in der Bauchhöhlenflüssigkeit erreicht wird. Unter den Bedingungen einer Peritoneallavage kommt es jedoch innerhalb von ca. 12 Stunden zum Absinken des verfügbaren Wirkstoffspiegels unter die Nachweisgrenze des Testkeimes. Durch die zusätzliche Verabreichung von Cefquinom in Gruppe 2 mit Spülflüssigkeit kann das Absinken der Cefquinomkonzentration vermindert bzw. verhindert werden. Die Cefquinomkonzentration lag in Gruppe 2 in der Peritonealflüssigkeit signifikant höher als in der Gruppe 1 und es werden bei den erreichten Konzentration mehr pathogene Keime einer Peritonitis erfasst.

Aim of the present study was to determine the concentration of cefquinome in the blood and in the peritoneal fluid in connection with the preventive treatment of the postoperative abdominal inflammation of the laparotomised horses. Finally followed the comparison between the results of the solitary systemic antibiotic therapy and the results of the systemic antibiotic therapy with additional local intraperitoneal antibiotic therapy. The various clinical and laboratory parameters were compared.The results of the current study assumed that the desired antibiotic concentration in both blood and abdominal fluid is achieved through single intravenous administration of cefquinom in the dose recommended by the manufacture. However, it requires about 12 hours under the circumstances of the peritoneal lavage to decrease the drug level below the MIC. In the second group, the decline of the drug concentration can be minimized through the addition of cefquinom to the administrated washing solution and even prevent that. The cefquinom concentration in the second group was significantly higher than that of the first group. It is reached in the second group more pathogens of peritonitis.
